In recent years,the incidence of breast cancer is increasing.According to 2008 data showing,the standardized incidence of female breast malignancy in the world reaches 39.0/100,000,ranking the first in female common malignant tumors.According to the latest statistic of GLOBOCAN 2012,there are up to 1.67 million new cases of breast cancer each year,and about 522,000 people die of breast cancer.Experts point out that hormone-dependent breast cancer accounts for at least 3/5 of all breast cancers,and the proportion of estrogen receptor-positive people is 3/5 to 3/4.Endocrine therapy is the most effective and basic treatment for the patients of breast cancer with hormone receptor-positive.However,in the process of receiving endocrine therapy,due to endocrine drugs,the balance of the endocrine environment of breast cancer patients is destroyed.And there will be a series of adverse reactions,such as hot flushes,palpitations,insomnia,osteoporosis,muscle pain,fatigue,weakness,irritability,emotional instability,vaginal dryness,ect,similar to perimenopausal syndrome,leading to multiple complex changes in physics and mental of patients,and even reduce the completion rate of standardized treatment and has a serious impact on patients and their families.Objective:To observe the clinical efficacy of Yi Shen Shu Gan tang of director Lin in improving the perimenopausal syndrome of kidney deficiency and liver stagnation type after endocrine therapy of breast cancer,and the effect of TCM on quality of life of breast cancer patients.To provide some clinical ideas for doctors to prevent and treat this kind of patients,and to provide clinical evidence for carring out targeted interventions and finding more effective methods by using TCM.Method:The study included 38 breast cancer patients with moderate-to-severe of perimenopausal syndrome of kidney deficiency and liver stagnation type after endocrine therapy.This research used self-control study method.On the basis of the original endocrine treatment,Chinese herbs of Yi Shen Shu Gan tang was used to intervene the patients,and observed the treatment for 30 days.After the end of the treatment,the changes of the observation indexes of the enrolled patients were recorded.Main observations included the common clinical symptoms(according to the modified Kupperman symptom score scale),emotional and psychological changes(according to the standard Greene climacteric scale),the changes in quality of life(according to the Functional Assessment of Cancer Therapy-Breast Cancer,FACT-B 4.0)and the TCM efficacy evaluation(according to the revised version of Chinese Menopause Rating Scale).The study used IBM SPSS Statistics 24 statistical software for data analysis.Result:In this study,38 patients were included and 2 patients were excluded.A total of 36 patients completed the efficacy observation.After intervention of traditional Chinese medicine,the seven clinical common symptoms of hot flashes,paresthesia,insomnia,agitation,fatigue,bone and joint pain,and palpitations were significantly improved compared with before,with statistical significance(P<0.01).The depression and suspicion,sensation of skin ants walking was significantly relieved,with statistical difference(P<0.05).In the psychological and emotional aspects,the anxiety and depressive symptoms of the patients improved significantly compared with before(P<0.01).The quality of life in physiological area,emotional area and functional area was improved than before and achieved satisfactory results with statistically significant differences(P<0.01).The overall adverse reactions of the patient’s in physical,psychological,and social dimensions were all significantly lower than before(P<0.01).Among the 36 patients,22 patients had effective treatment,and 14 patients had no obvious effect.The effective clinical remission rate of Yi Shen Shu Gan tang of professor Lin in improving the perimenopausal syndrome of kidney deficiency and liver stagnation type after endocrine therapy is 61.11%.In addition,the use of Yi Shen Shu Gan tang was safe and no abnormal liver and kidney function was detected in the patients enrolled.Conclusion:Yi Shen Shu Gan tang of professor Lin can be used in the breast cancer patients of the perimenopausal syndrome of kidney deficiency and liver stagnation type after endocrine therapy.It has obvious therapeutic effects on physical symptoms and psychological conditions,and has good clinical efficacy.It is worthwhile to increase the sample size and conduct long-term follow-up to clarify the mechanism of the role of Chinese medicine and the influence of Chinese Medicine on the patients’survival.It is helpful to guide clinicians in combination with the TCM syndrome differentiation theory,explore suitable population for other TCM treatment methods,and bring into play the unique advantages of TCM treatment.
