| Objective: We aim to study the effect of the repetitive pain stimulation on the prognosis and cognitive sleep of patients with acute mild ischemic stroke within 24 hours after standard treatment Methods: We retrospectively examined 206 patients with acute ischemic stroke within 24 hours who consecutively were admitted to the First Hospital of Chenzhou from January 2016 to September 2017.All patients underwent standardized clinical assessment and cerebral MRI(including general sequencing and susceptibility-weighted imaging).And all patients were accepted the standard treatment of mild ischemic stroke in 24-hour.And all patients were randomly divided into 2 groups.101 patients in the experiment group which was accepted the standard treatment and repetitive pain stimulation which chosen the inner surface face of auricle by using the Erxue-bead to meeting requirement which requirement of VAS of 4-6 point.The repetitive pain stimulation is 5-6 s/time and 15 time/day by using the Erxue-bead and 105 patients in the control group were accepted the standard treatment.Patients’ Fugl-Meyer Assessment,modified rankin scale,NIH Stroke Scale,Montreal Cognitive Assessment,Pittsburgh Sleep Quality Index,The Hamilton Depression Rating Scale and Activities of Daily Life Assessment were summarized in 0-day,14-day,30-day,90-day.Patients’ characteristics were summarized for each group separately.We did statistical comparison between two groups of t-test,Chi-square and Cox function to identify the relative risk factors.Results: 1.According to achieved data from 206 patients with mild ischemic stroke in 24-hour,we found that 9 patients of the experiment group were poor prognosis accounting for 8.9%,and 12 patients of the control group were prognosis accounting for 11.4%.Repeated pain stimulation was an independent factor affecting the prognosis of patients with mild ischemic stroke within 24 hours(p=0.0139).And compared with the control group,the prognosis rate of patients in the intervention group was lower than the control group,HR=0.720(95%Cl: 0.466-1.113).2.According to achieved data from 206 patients with mild ischemic stroke in 24-hour,we found that the cognitive function of 10 patients of the experiment group was declined accounting 10.0%,and the cognitive function of 22 patients of the control group was declined accounting for 21.0%.There was no statistically significant difference in the risk of cognitive impairment(p=0.995)within 90 days of patients with mild ischemic stroke within 24 hours.And compared with the standard medical treatment,the risk of cognitive dysfunction in the intervention group was the same as the control group,HR=1.000(95% Cl: 0.862-1.161).3.According to achieved data from 206 patients with mild ischemic stroke in 24-hour,we found that the sleep quality of 15 patients of the experiment group was declined accounting 14.1%,and the sleep quality of 55 patients of the control group was declined accounting for 52.3%.Repeated pain stimulation was an independent factor(p<0.0001)that affected the quality of sleep within 90 days of mild ischemic stroke patients within 24 hours.And compared with the control group,the risk of sleep quality was lower in the intervention group than in the control group,HR=0.1526(95%Cl: 0.1083-0.2151).4.According to achieved data from 206 patients with mild ischemic stroke in 24-hour,we found that 13 patients of the experiment group becoming risk of depression accounting 14.1%,and 63 patients of the control group becoming risk of depression accounting 60.0%.Repeated pain stimulation was an independent factor(p<0.0001)that affected the risk of depression in patients with mild ischemic stroke within 24 hours.And compared with the control group,the risk of depression in the intervention group was lower than the control group,HR=0.1346(95%Cl: 0.09979-0.1816).Conclusion: Within 24 hours of onset of mild ischemia stroke patients in repeated pain stimulus can reduce defective rate prognosis within 90 days,improve the sleep disorders within 90 days and reduce the incidence of depression within 90 days,but it can’t affect cognitive function in probability within 90 days. |
