Public Policy Analysis Of Nanjing Physicians' Multi-sited Practice

From time,use the theory of public policy implementation to classify the problems of influencing physicians involved in multi physician practice,and from the perspective of the classification of the various aspects involved,they were held in this paper:firstly,the policy itself,research insufficiency,the operability is not strong,did not consider the balance of interests of various parties,especially to master a lot of health human resources in the interests of the medical institution makes policy at the beginning of the execution is struggling,always difficult to open the situation.Secondly,policy enforcement agencies,the administrative department of health will never do more practice included in the assessment of project,and the public medical institutions is both the executor of the policies and policy object,the inevitable in the process of carrying out the policy,select favorable executive way.Third,the policy object,the large public medical institutions themselves have a large number of health human resources,the demand for multi-point practice is not strong;Private medical institutions and grassroots medical institutions are strong demand,but the private medical institutions have rich physical capital,so in the game in exchange for a small number of physicians involved in more practice,but on the whole,led to the execution of the policy deviation;Practicing physicians,for various reasons,sometimes chose to go through the offending "hole",instead of opting for regular practice.The patient's desire to implement the policy is not strong,which is related to the lack of the classification referral system and the long-term medical treatment habits of the large hospital.Fourthly,policy resources,grassroots medical institutions because of the lack of investment for a long time,leading to medical equipment,medical technology,medical environment,in recent years,with the increase of national health input,situation changed,but still with large public medical institutions,there is a big gap It is not timely to update the information about doctor's multi-point practice policy,and the inherent authority of large public hospitals has also affected the participation of doctors in multi-point practice.Five,policy environment,policy and practicing doctors more registration regulations don't join at the early stage of the policy implementation,our country health existing in large public medical institutions and personnel system for the characteristics of the object itself,as well as medical disputes exist in the process of long-term disposal difficulties,further hindered the practitioners more the implementation of the policy.Finally,based on the reasons that influence physicians' participation in multi-point practice,this paper puts forward a strategy to promote the multi-point practice of physicians:to increase government investment in medical and health reform;health personnel system;diagnosis and treatment to combat illegal acts according to law;clear medical liability share in the responsibility;strengthen policy implementation and evaluation;improve grading treatment the referral system and further simplify more practice procedures.
