Objective:the present study by observing the changes of plasma melatonin levels in patients with acute ischemic stroke to assess the clinical significance of melatonin and its relationship with the risk factors of cerebral infarction.Methods:1、Research object:131 cases of acute cerebral infarction within 24 hours of onset and hospitalized in Department of Neurology,Second Hospital of Jilin University from June 2017 to December 2017 were selected as case group and their clinical data were collected including previous history of hypertension,diabetes mellitus,smoking and drinking,as well as body mass index,routine blood tests,imaging results of brain and vascular of head and neck.According to the different parts of infarct,the patients with acute infarct of brain were divided into subgroups including frontal lobe,temporal lobe,occipital lobe,parietal lobe,basal ganglia,thalamus,cerebellum,brain stem,radiation crown and semi-oval center.49 healthy people in the same period is the control group.2、Plasma melatonin detection:Radioimmunoassay was used to detect the plasma melatonin levels in the case and control groups.we compared the plasma melatonin levels in each group and subgroup.The relationship of melatonin and clinical data of case group(history of hypertension,diabetes mellitus,smoking and drinking,as well as body mass index,biochemical parameters,such as fasting blood glucose,Blood lipids,Cystine,high-sensitivity C-reactive protein,uric acid,cystatin C,cholinesterase,alkaline phosphatase,monoamine oxidase)were analyzed.The data were processed by statistical software of SPSS18.0.Results:1.In the case group,there are a total of 131 patients and their average age were 60.41 ± 7.99 years,of which 100 were males and 31 were females;control group includs 49 healthy people.the average age of them wes 58.71 ± 7.29 years,of which 30 were males and 19 were females.the difference of gender and age between two groups above is no significance(p> 0.05).2.In the case group,the lesion site:semi-oval center was 10(7.6%);the parietal lobe was 11(8.4%);the frontal lobe was 12(9.2%);radiationcrown was 15(11.5%);basal ganglia was 18(13.7%);brain stem was19(14.5%);temporal lobe was 8(6.1%);thalamus was 14(10.7%);cerebellum was 10(7.6%);occipital was 14(10.7%);There was no significant difference in sex and age of subgroup(p> 0.05).3.Compared with the control group,plasma melatonin levels in patients with cerebral infarction decreased significantly(p <0.01),and there was significant difference of plasma melatonin levels in patients with different site cerebral infarction(P> 0.05).The melatonin values between women and men in both case group and control group is noo significant difference(p>0.05).4.cerebral infarction Patients with a history of hypertension had higher levels of melatonin than those without history of hypertension(P<0.05).Whether they have history of smoking,drinking,diabetes or not,There was no significant difference of the plasma melatonin level(P>0.05).5.Whether the levels of total cholesterol,low density lipoprotein,high density lipoprotein,homocysteine,high sensitivity C-reactive protein,fasting blood glucose,triglyceride,lipoprotein a and uric acid in patients with cerebral infarction is normal or increased.There was no significant difference of the plasma melatonin level(P> 0.05).5.The level of plasma melatonin in patients with cerebral infarction was negatively correlated with the degree of atherosclerosis(rho =-0.341,<0.01)and the age(rho=-0.179,p <0.05).There were no significant correlations between melatonin and other biochemical parameters Related to cerebral infarction(BMI,cystatin C,apolipoprotein A,apolipoprotein B,lipoprotein a,monoamine oxidase and fibronectin)(P> 0.05).Conclusion:1.Plasma melatonin levels of patients with acute cerebral infarction is decreased,and the melatonin levels are related to the location of cerebral infarction.2.cerebral infarction Patients with a history of hypertension had higher levels of melatonin than those without history of hypertension.3.The level of plasma melatonin in patients with cerebral infarction was negatively correlated with the degree of atherosclerosis and the age.
