| Purpose:By comparing the atherosclerotic acute cerebral infarction patients with asymptomatic atherosclerosis patients with vascular stenosis degree,collateral circulation between set up or not,the plaque stability,vascular stenosis rate and proportion of vulnerable plaques of the former may be higher than the latter,collateral circulation formation may be less than the latter,and the plaque stability of the former meybe worse.We can detect peripheral blood VEGF,ox-LDL and MMPS levels to reflect patients’ degree of collateral circulation formation and plaque stability.It’s helpful for us to analyze the clinical condition.We can give intervention to promote the generation of collateral circulation,increases plaque stability,control the related risk factors,in order to reduce cerebrovascular events,reduce the mortality and morbidity as soon as possible.Methods:64 patients with atherosclerotic acute cerebral infarction of the experimental group 1(group A),57 patients with asymptomatic atherosclerosis as experimental group 2(group B),all fit into the group standards: atherosclerotic plaque,hemadostenosis or blockage were found through cerebrovascular examination(color doppler ultrasound,MRA and CTA);And 10 cases of healthy people without atherosclerosis as blank control group(group C):the blood vessels are normal examined by color doppler ultrasound,MRA and CTA.All cases in group A are inconformity with the guide of diagnosis and treatment of acute ischemic stroke in China 2014 diagnostic criteria,confirmed by brain CT or MRI without active bleeding or bleeding tendency,excluded the onset age was between 18-80 years old,exclude active ulcer,excluded patients with severe heart lungs hepatorenal dysfunction.To clear group A and group B patients with supplying artery atherosclerotic stenosis degree and through the comparison for both vascular stenosis rate,analyzed its relationship with acute cerebral infarction.According to the condition of blood vessel within each group,all divided into three subgroups: patch/mild stenosis group,moderate stenosis group,severe stenosis /block group.Statistics and contrast the proportion of vulnerable plaques in both groups,to investigate the relationship between the nature of plaque and acute cerebral infarction occurred.Within 24 hours Each patient was also extracted elbow venous blood 5 ml in a test tube equipped with anticoagulants;After centrifuging(3000 r/min,15 min)blood samples to return in the EP test tubes,saved in-80 ℃ refrigerator.Contrasting the blood levels of VEGF、ox-LDL、MMP-9 between the patients of group A,B with atherosclerosis and group C,to discuss the relationship between VEGF、ox-LDL、MMP-9 and atherosclerotic.Under the condition of the same blood vessels,to contrast group AB VEGF levels.Statistics and contrast presence of differences between the three groups of peripheral blood ox-LDL,MMP-9 level.Results:1.The patients of group A with vascular lesions degree is given priority to with moderate stenosis,severe stenosis/block,on the contrary,the patients of group B with mild stenosis.Comparing the hemal stricture rate of both groups,it’s having statistical significance(P=0.001).2.The proportion of vulnerable plaques and stable plaques in blood vessels of group A patients respectively are 50.0%,78.1%;the proportion in group B both are 24.6% and 86.0%.Proportion of vulnerable plaques in group A is significantly higher than group B(50%>24.6%,P=0.004).The proportion of stable plaques in both groups were similar(respectively78.1% and 86.0%,P=0.266).3.The levels of peripheral blood VEGF、ox-LDL、MMP-9 in group A are significantly higher than group C(P=0.01、P<0.001、P<0.001),that of group B is also significantly higher than group C(P=0.005 、P=0.012、P=0.003).4.Under the condition of the same blood vessels,that is to say,respectively in plaque/mild stenosis group,moderate stenosis group,severe stenosis /block group,the levels of peripheral blood VEGF in group B were higher than that of group A(P=0.012,P=0.028,P<0.001).5.Peripheral blood ox-LDL levels gradually reduce between the three groups(group A was higher than group B,group B was higher than group C,P<0.001),with statistical significance.Peripheral blood MMP-9levels of group A higher than that of group B,group B was higher than group C,the difference is compared between three groups with statistical significance(P<0.001).And the levels of peripheral blood MMP-9,ox-LDL compared between any two means were all P<0.05.Conclusions:1.The patients who have a high level of ox-LDL,MMP-9 and the low level of VEGF are more likely to happen in acute cerebral infarction.2.The patients with atherosclerosis peripheral blood VEGF,ox-LDL,MMP-9 level are obviously higher than that of healthy human,suggest that the above factors may be related to the formation of atherosclerosis.3.Detecting of peripheral blood VEGF,ox-LDL,MMP-9 levels maybe helpful to find cerebrovascular problem timely,we can give related intervention in order to reduce cerebrovascular events,reduce the morbidity and mortality as soon as possible. |
