The Clinical Comparison Between Complete Thoracoscopy And The Traditional Open-chest Atrial Septal Defect Repair

Objective:Collected clinical data and through the data analysis will be entirely under the thoracoscope and median thoracotomy compared the clinical effect of repair of atrial septal defects,analysis the advantages and disadvantages of two kinds of operation method,provide guidance for clinical choice.Methods:Selection in October 2015 to October 2017 in the first affiliated hospital of xinjiang medical university heart surgical treated 182 cases of patients with atrial septal defects,the two groups of patients in accordance with the surgical procedure is divided into two groups: the thoracoscope group 102 examples,the center of the traditional open heart surgery group 80 cases.The differences between the two groups in operation time,extracorporeal circulation time,ventilator assisted time,ICU retention time,postoperative drainage and postoperative hospital stay were compared.Results: all the patients completed the surgery successfully,and there were no serious complications in the perioperative period and residual shunt after surgery.There were no statistically significant differences in gender,weight,age,right atrial size,cardiac function,pulmonary arterial pressure between the two groups before surgery(P > 0.05).Group compared with the traditional thoracotomy surgical group,completely thoracoscope surgery time and extracorporeal circulation time is longer than the median thoracotomy group,p < 0.05,and the breathing machine auxiliary time,ICU stay time,had the flow rate and postoperative hospitalization days were short in the middle of traditional thoracotomy group,p < 0.05,the difference was statistically significant.Conclusion: the curative effect is completely thoracoscope interatrial septum defect repair,safe and feasible,and the learning curve is longer,higher request to the performer,but compared two groups of operation method,thoracoscope completely repair of atrial septal defects(less surgical trauma,less bleeding,postoperative hospitalization days shorter,without the destruction of the bone structure,good cosmetic effect,is worth promoting.
