| Objective:To explore the change of serum IGF-1 and insulin resistance in type 2 diabetes and T2 DM complicated with non alcoholic fatty liver.To define the possible association of serum IGF-1 in T2 DM and NAFLD.Methods:A retrospective study of 166 cases who were diagnosed with type 2 diabetes in the Second Affiliated Hospital of Dalian Medical University from February 2017 to December 2017.Main outcome measures included liver imaging examination and serum IGF-1,fasting blood glucose(FBG),hemoglobin A1c(Hb A1c),fasting plasma insulin(FINS),C peptide(C-peptide),fasting plasma insulin(FINS),aspartate aminotransferase(AST),alanine aminotransferase(ALT),blood lipids: glycerin three greases(TG),cholesterol(TC),high density lipoprotein the cholesterol(HDL-C)and low density lipoprotein cholesterol(LDL-C).The insulin resistance index(HOMA-IR)=FBG×FINS/22.5.Results:The level of serum IGF-1 in type 2 diabetes mellitus with NAFLD was significantly lower than that in patients with simple diabetes(P <0.05),which suggested that serum IGF-1 level decreased with the severity of liver histology.The level of serum IGF-1 had inverse correlation with BMI,FPG,TC,TG,LDL-C,HOMA-IR,while had no obvious relativity with Hb A1 c,sex.Taking the level of serum IGF-1as dependent variable,BMI,fasting blood glucose,cholesterol,triglyceride and HOMA-IR as independent variables for multivariate linear stepwise regression analysis.It was obtained that HOMA-IR is an independent factor of IGF-1.Conclusions:1.Patients with T2 DM and NAFLD have lower serum IGF-1,more severe insulin resistance,glucose and lipid metabolism disorders,as the severity of liver disease,serum IGF-1 levels were significantly lower.2.The serum IGF-1 levels have negative relationship with BMI and HOMA-IR,which suggesting that obesity-related insulin resistance may be related to IGF-1,HOMA-IR is the independent factor affecting serum IGF–1. |
