Objective:To explore the feasibility and clinical effect of using mesh split-thickness skin autograft combined continuous vacuum sealing drainage technology to repair refractory wounds due to a variety of causes.Methods:Summarize 30 cases of refractory wounds patients due to skin and soft tissue defect in our Burn Plastic Surgery in March 2013 to April 2015.30 cases of patients are divided into two groups.Application two methods treatment:mesh blade thickness skin autograft combined VSD techniques therapy(group A)and directly mesh split-thickness skin autograft after the wound is given a debridement processing mesh split-thickness skin autograft(group B).Observe and record two groups of patients’ body temperature and white blood cell count,secretion culture results,the healing of the wound;comparison between the two groups of wound dressing,operation rate and postoperative hospitalization days and the postoperative complications;compare two groups of cases of clinical therapeutic effect.Results:1.A,B two groups of patients’body temperature,white blood in postoperative day 1,3,5,7 had no statistical difference(P>0.05).2.A group of patients with wounds bacterial culture turn rate,switching frequency,operations,skin graft survival rate for the first time,postoperative complications,postoperative hospitalization days were 70%,5.5 times,1.2 times and 86.7%,3cases,10.8 days.3.B group of patients with wounds bacterial culture turn rate,switching frequency,operations,skin graft survival rate for the first time,postoperative complications,postoperative hospitalization days were 55.6%,7.6 times,1.6 times and 80%,4cases,12.6 days.4.A,B two groups of patients with wounds bacterial culture turn rate,switching frequency,operations,skin graft survival rate for the first time,postoperative complications,postoperative hospitalization days,the difference was statistically significant(p<0.05).Conclusions:First,mesh blade thickness skin autograft combined the VSD technique to repair wounds in a timely manner,at any time to remove subcutaneous hemorrhage,effusion and necrotic tissue,reduce the wound infection rate,improve the skin graft survival rate,reduce the skin necrosis caused by skin graft site infection rate.Second,mesh split-thickness skin autograft combined the VSD technique can reduce the number of wound dressing,operation frequency,accelerate wound repair,shorten the length of hospital stay,etc.Third,the method use mesh split-thickness skin autograft combined VSD technique to repair skin and soft tissue defect wound is feasible,and the clinical effect is good,worthy of clinical popularization and application. |
