[Objective]Discussion:1.To explore the application of three-dimensional visualization technique in reconstruction of hepatic-vascular-biliary-stone three-dimensional topographic map model to preoperative assessment of complicated hepatolithiasis;2.To explore the use of three-dimensional visualization technology for reconstruction of Hepatolithiasis image model the application of accurate calculation of calculus Total product in the postoperative evaluation of complicated hepatolithiasis operation.[Methods]To collect the clinical data of 30 cases of complicated hepatolithiasis in Hunan People’s Hospital from December 2017 to March 2018,all patients use three-dimensional visualization technique to reconstruct the three-dimensional imaging model of liver-blood vessel-biliary-stone,and analyze it to guide the development of the Operation plan.To preoperative assessment;All patients before and after operation using three-dimensional visualization technology to reconstruct the image model of hepatolithiasis,through the stone model to calculate the total volume of stone,the final calculation of postoperative immediate stone volumetric clearance rate,postoperative calculus volume residual rate,The new clinical quantitative indicators,as well as the feasibility of a new quantitative index of scientific research,were analyzed as the evaluation of surgical effectiveness and prognosis of complex hepatolithiasis.[Results]The three-dimensional topographic map of liver-vessel-biliary-calculus was successfully reconstructed in 30 patients before operation,all patients were 1.It was clear enough to show the shape of the intrahepatic vessels,and to segment the liver individually;2.It can clearly show the distribution of calculus within the biliary tract system,3.The stenosis of the bile duct can be clearly defined and the extent of it;4.It can accurately show the relative spatial position of hilar bile duct and adjacent vessels;5.It can be accurately plotted from the biliary tract into the liver site to the target stone location required to go through the path map;6.It can accurately locate the closest distance and path of the target bile duct and stone to the surface of the liver;7.It can clearly show the relative spatial position of intrahepatic target bile duct and blood vessel;8.The relative position relationship between the hepatic segment,the distribution of blood vessel and the target bile duct and stone was clearly demonstrated.9.All the results can be used to calculate the total volume of hepatolithiasis after operation.[Gonclusion]1.Using three-dimensional visualization technology to reconstruct the three-dimensional map of the liver-blood vessel-bile duct-stone,for preoperative assessment is feasible;2.It is feasible to use three-dimensional visualization technique to reconstruct hepatolithiasis model and to evaluate the clinical operation effect of complicated hepatolithiasis.
