The First Section: Status of coagulation function in hospitalized patients with burns(adults)Objective: To observe the status of coagulation function in patients(adult)with different degrees of burns during hospitalization and to provide suggestions for the treatments of the burned patients.Methods: The basic information of the burned patients(adults)admitted to the Burn Department of the 174 Hospital of Chinese People’s Liberation Army from July 2016 to December 2017 was collected,including the gender,age,burn area,presence of inhalation injuryand presence of III degree burns,then the ABSI score was evaluated for patients.According to TBSA,the patients were divided into mild to moderate burn group and severe burn group.According to ABSI score,the patients were divided into low ABSI group,middle ABSI group and high ABSI group.Randomly selected health check-ups without blood system diseases were set as a healthy control group at the same time.The tests of thromboelastograms,coagulation functions and blood routine were performed for the patients when they were admitted into hospital immediately,then the status of coagulation function of the patients were analyzed.Results:According to TBSA,the APTT,TT,R,and K in the mild to moderate burn group and the severe burn group were lower than those in the healthy control group.The DD and PLT in the mild to moderate burn group and the severe burn group were higher than those in the healthy control group.The DD and FDP in the severe burn group were higher than those in the healthy control group and the mild to moderate burn group.The MA of the severe burn group was lower than that of the healthy control group and the mild to moderate burn group.The DD of the severe burn group was higher than that of the mild to moderate burn group.The difference was statistically significant(P<0.05).According to ABSI,the APTT,TT,R,and K in the low ABSI group,middle ABSI group and high ABSI group were lower than those in the healthy control group.The DD and PLT in the low ABSI group,middle ABSI group and high ABSI group were higher than those in the healthy control group.The APTT in the middle ABSI group and high ABSI group was higher than that in the low ABSI group.The DD in the high ABSI group was higher than that in the low ABSI group and the middle ABSI group.The FDP in the high ABSI group was higher than that in the healthy control group,the low ABSI group,and the middle ABSI group.The MA in the high ABSI group was lower than that in the healthy control group,the low ABSI group,and the middle ABSI group.The difference was statistically significant(P<0.05).Conclusion:The coagulation function of burn patients has undergone certain changes,which mainly manifested in a certain degree of hypercoagulability and hyperfibrinolysis,clinical attention must be paid to the treatment of burn patients.The Second Section: Application of clinical transfusion in the treatment of burnned patients(adults)Objective: To explore the application of different blood products in the clinical transfusion treatment of burnned patients and make a more rational clinical transfusion treatment plan for the patients burnning.Methods: The basic information of the burned patients(adults)admitted to the Burn Department of the 174 Hospital of Chinese People’s Liberation Army from July 2016 to December 2017 was collected,including the gender,age,burn area,presence of inhalation injuryand presence of III degree burns,then the ABSI score was evaluated for patients.According to TBSA,the the status of coagulation function of the patients,blood transfusion therapy was performed to achieve the goals of increasing the hemoglobin,platelet or correcting coagulation function for the patients.The information,such as intraoperative blood loss and blood transfusion during hospitalization,was collected to analyze the relationship between the blood loss and the blood transfusion of patients with different burn levels.Results:According to TBSA,the blood loss,red blood cell,plasma and platelet usage in the severe burn group were higher than those in the mild to moderate burn group with the difference statistically significant(P<0.05).According to ABSI,the blood loss,red blood cell,plasma and cryoprecipitate use in the middle ABSI group and the high ABSI group were higher than those in the low ABSI group.The high ABSI group had a higher useage of platelet than that of the low ABSI group and the middle ABSI group with the difference statistically significant(P<0.05).Conclusion:Blood transfusion therapy is of great significance in the clinical treatment of burn patients.With the increase in the degree of burns,the use of blood products will increase accordingly.For blood transfusion treatment of burn patients,preparation of corresponding blood products should be done. |
