Background: To summarize and analyze the clinical characteristics and prognosis of patients with acute posterior circulation infarction and bilateral vertebral/basilar artery stenosis/occlusion.Methods: Clinical data of all patients diagnosed with acute posterior circulation infarction and bilateral vertebral/basilar artery ≥70% stenosis/occlusion in hospital from January 2015 to August 2017 was retrospectively collected.The vascular risk factors,initial symptoms,treatment and progression were analyzed,and the 90 th day modified Rankin Scale(mRS)was followed.mRS >2 was defined as poor prognosis.Results: Twenty-eight patients were included,the most common initial symptom was isolated dizziness/vertigo,accounting for 57.1%.The signs and symptoms of 15 patients(53.6%)aggravated,9 cases aggravated ≥2 times.Fifteen patients(53.6%)had poor prognoses.The ratio of stage 3 hypertension,aggravation,and aggravation ≥2 times were higher in mRS >2 group than mRS ≤2 group(P<0.05).The ratio of stage 3 hypertension,hyperlipidemia,initial symptom of isolated vertigo/dizziness,poor prognosis and mortality were significant higher in aggravation ≥2 times group(P<0.05)than that in aggravation <2 times group.Four patients’ basilar artery were indistinct on artery images,100% of which had poor prognoses,100% of them had poor prognoses,and their mortality rate was higher(P<0.05).Conclusions: Patients with acute posterior circulation infarction and bilateral vertebral artery/basilar artery severe stenosis/occlusion are often onset with isolated dizziness/vertigo.Patients with stage 3 hypertension,indistinct basilar artery imaging or onseting with isolated dizziness/vertigo are prone to aggravate.Patients who aggravate more than one time often have poor prognosis and high mortality.Patients with indistinct basilar artery imaging have a high mortality rate.
