Objective: To observe the correlation of the difference of interankle systolic blood pressure(IAND)with microalbuminuria(MAU)and left ventricular hypertrophy(LVH)in essential hypertensive patients,and to provide a valuable testing base for early detection of target organ damage in hypertensive patients.Methods: 307 essential hypertensive patients were selected,including 143 males and164 females,with an average of 61.03±12.74 years.Blood pressure,height and weight were measured,and body surface area(BSA)、body mass index(BMI)、 IAND were calculated.bvenous blood determination:high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C),total cholesterol(TC),triglycerides(TG),creatinine(Cr),and then take an early morning fasting urine to measure urinary albumin/creatinine(UACR).All patients underwent echocardiography and left ventricular mass index(LVMI)was calculated.(1)According to LVMI and UACR results,patients with essential hypertension were divided into four groups: group A:simple hypertension group;group B: hypertension with LVH;group C: hypertension with MAU;group D: hypertension with LVH and MAU.IAND difference between the two groups were compared,and the relationship between IAND difference and UACR and LVMI was analyzed in hypertensive patients.(2)According to IAND,patients with essential hypertension were divided into three groups: A1 group: IAND<10mmHg;B1group: 10mmHg≤ IAND <15mmHg;C1: IAND ≥ 15 mmHg.The incidence of LVH and the detection rate of MAU in the three groups were compared,respectively,and the predictive value of the difference in blood pressure between IAND on hypertension with LVH and/or MAU was analyzed.result: 1.There was difference in age and gender between hypertension with LVH and/or MAU and simple hypertension(P<0.01).Group D(76.64±5.14 years old)>C group(73.63±4.66 years old)> B.Group(57.28±6.39 years old)>A group(53.05±8.99 years old),the difference was statistically significant.The proportion of males in group C(31.5%)was higher than that of group B(10.3%).The proportion of females in group B(89.7%)was higher than that in group A(40.4%).The proportion of women in group D(73.8%)was higher than that of group B.In group A(40.4%),the difference was statistically significant.2.There was a difference in IAND between hypertension and LVH and/or MAU and hypertension alone.Among them,C(16.94±14.03mmHg)and D(15.12±12.56mmHg)were significantly higher than those of B.The group B(7.86±5.798mmHg)was higher than the A group(3.04±5.42mmHg).The difference was statistically significant;there was no difference between the C and D groups.3.Age differences existed among different IAND groups,among which the ages of C1(67.59±10.59 years)and B1(71.00±10.58 years)were significantly higher than those of the A1 group(54.53±10.45 years),and the difference was statistically significant.There was no significant difference in the age of the C1 group.There was a difference in the incidence of LVH and MAU among different IAND groups.Among them,the incidence of LVH in the C1 group(58.1%)and B1 group(54.7%)was significantly higher than that in the A1 group(15.3%).The incidence of MAU in the C1 group(83.9%)and B1 group(73.3%)was significantly higher than that in the A1 group(7.1%).The difference was statistically significant;there was no statistical difference between the B1 group and the C1 group.4.Logistic regression analysis showed that IAND is an independent risk factor for hypertension combined with LVH and MAU.The difference was statistically significant.In primary hypertension,abnormal IAND of those with IAND was 1.865 times that of normal;MAU occurred 1.201 times normal.Conclusion: 1.The IAND of essential hypertension with LVH and/or MAU was significantly higher than that of patients with pure primary hypertension.2.There is an age difference in IAND,and older IAND are older;IAND abnormalities can be used as independent risk factors for predicting LVH and MAU.
