OBJECTIVE:The purpose of this article is to explore liver fat content(LFC)and related risk factors in people with different glucose metabolism,and to investigate the relationship between bone mineral density(BMD)and LFC in postmenopausal women with type 2 diabetes mellitus(T2DM)and those with T2 DM in men over the age of 50.METHODS:A total of 288 in-patients or healthy volunteers containing140 males and 148 females who visited the First Affiliated Hospital of Guang xi Medical University from May 2015 to April 2017 were selected.The data collection of subjects including the basic clinical data,blood biochemical indexes,LFC(Liver fat content was quantified via an ultrasound quantitative method),bone mineral density and so on.In the first part,according to the results of OGTT and medical history,the subjects(n=288)were divided into 4groups: type 2 diabetes group(T2DM group,n=122),newly-diagnosed type 2diabetes mellifluous group(New DM group,n=75),impaired glucose regulation group(IGR group,n=33),normal glucose tolerance group(NGT group,n=58).The subjects were divided into 4 groups according to LFC quadrille.In the second part,choose the postmenopausal women with T2DM(n=78)and the men with T2 DM who were older than 50 years(n=75)as study subjects.The subjects(n=153)were divided according to the LFC fertile and gender.RESULTS:In the first part,1.Age,waistline(WC),FPG,2h PG,HbA1 C,TG,BMI,T-CHO were highest in New DM group or T2 DM group,the difference was statistically significant compared with the NGT group.HDL-C in the NGT group was the highest,the lowest in the New DM group,the difference was statistically significant.2.The detection rate of NAFLD in NGT,IGR,New DM,T2 DM four groups was 44.81%,78.78%,90.66%,72.13%(P<0.01).The LFC of IGR group,New DM group and T2 DM group was higher than that of NGT group(P < 0.01),but no significant difference between the three groups.3.With the increase of LFC,FPG,2h PG,HbA1 C,TG,BMI,SUA showed an increasing trend(P<0.05),HDL-C showed a downward trend(P<0.01).4.Pearson correlation analysis showed that the LFC was positively correlated with SBP,BMI,WC,SUA,TG,FPG,HbA1 C and negatively correlated with HDL-C(P<0.05).5.Multiple linear regression analysis showed that WC and TG entered the regression equation with the LFC as dependent variable.The second part,1.In T2 DM group,with the increase of LFC,BMI,WC,TG increased,AST/ALT decreased(P<0.05).2.According to the situation of bone mineral density grouping,the results showed the liver fat content of osteoporosis group is less than the bone loss group,but there was no statistically significant difference between the two groups.3.The left femoral neck and hip BMD increased with the LFC.In postmenopausal women with T2 DM,the femoral neck and left hip BMD value increased with the increase of LFC,the highest and lowest group was statistically significant(P < 0.05).Analysis ofcorrelation was performed between the LFC and the BMD.In T2 DM group of postmenopausal women,the results showed that age,menopausal age,BUN were negatively correlated with the BMD of femoral neck and the left hip,and LFC were positively correlated(P< 0.05).Age were negatively correlated with the BMD of L1-L4,LFC were no correlation with L1-L4 BMD(r=0.099,P>0.05).In T2 DM group of men over the age of 50,the results showed that weight were positively correlated with the BMD of femoral neck,the left hip and L1-L4,and negatively correlated with HDL-C.The BMD of left hip and femoral neck were negatively correlated with FT3,Apo AI.The BMD of L1-L4 was positively correlated with BMI,WC,hemoglobin(Hb)(P < 0.05).4.The multiple linear regression analysis was carried out with the BMD of different sites as the dependent variables.In the group of postmenopausal women with T2 DM,the results showed that the age,LFC entered into the regression equation(P<0.05).In the group of man with type 2 diabetes over the age of 50,HDL-C,FT3,Hb and weight entered into the regression equation(P<0.05).CONCLUSIONS: 1.With the aggravation of glucose metabolism disorder,liver fat deposition was obvious,and the prevalence of NAFLD increased.TG,waistline,low AST/ALT were independent risk factors for NAFLD.2.Age is a risk factor for BMD and LFC may be a protective factor for BMD in postmenopausal women with T2 DM.3.HDL-C and FT3 may be the risk factor for BMD and body weight,Hb may be the protective factors for BMD in men over the age of 50 in patient with T2DM.
