| Purpose:Through the establishment of minipig animal model of cleft palate,group to perform traditional soft tissue flap repair and distraction osteogenesis,holding this two ways to repair cleft.A comparative study of the influence on the subsequent development of the jaw with distraction osteogenesis for treatment of cleft palate.Methods:2 months old male 21 miniature pigs have been randomly divided into A,B,C three groups(A group of six,group B 6,group C 9).All the animals were made into animal models of cleft palate which is made by surgical methods.No repair is done with group A,soft tissue flap repair is done with group B,and DO to closed fracture with group C.The tension of group C was done over the same period as group A/B.Monthly CBCT examination was done for 6 months to observe the jaw development situation.In addition,after stretching one animal was randomly extracted from group C in 2 w,4 w and 8 w to get the tissuesamples in tension zone,using HE staining to observe the bone tissue formation.After the observation period,animals were sacrificed,and the area of mucosal tissue was cut to observe the scar formation by Masson staining.Results:1.Animal cleft palate model is established in this paper,two kinds of operative methods are successful for repair of cleft palate.Postoperative wound healing is good.The animals did not die due to intraoperative bleeding and postoperative infection.2.Distraction osteogenesis correct cleft by intramembrane osteogenesis.After 2w of DO angiogenesis is observed with osteoid formation;After 4w of DO early bone trabecular formation with visible connected into network is seen;8w after DO new bone mineralization is seen which mature further.3.Mucosal scarring is seen,the mucous membrane of scar after cleft palate repair soft tissue flap group is more serious than the stretching group.4.There was no significant difference between the three groups about the jaw development.But,within the group itself,group B after surgery jaw development is relatively slow,then group A and group C keep a certain growth rate.Conclusion:The repair of cleft palate is aimed at the improvement of the functionality and aesthetics.The repair of soft tissue flap resulted in severe scar formation and inhibited the subsequent development of jaw bone.DO technique for cleft palate repair do not influence the development of jaw,besides realize the true meaning of cleft closure.DO technique can be used as a new way torepair cleft palate. |
