Complications Analysis And Preventive Measures Of Gynecological Laparoscopic Surgery

Objective: to explore the causes and preventive measures of complications of gynecological laparoscopic surgery.Methods: This essay retrospectively analyzed 4292 cases of gynecologic laparoscopic surgery clinical data in Huangshan People’s Hospital from 2013 to 2017,summarized related reasons of 85 cases of complications,and discussed complications prevention measures.Results: 1.There were 4292 cases of laparoscopic surgery in Huangshan People’s Hospital from January 2013 to December 2017 with 85 complications,and the incidence of complications(1.98%)is slightly higher than the literature report.As time goes on,the figure of laparoscopic surgery complicationsis generallyon the decline,especially that in 2013 is highest at 2.9%,with a lowest record 1.23% in 2017.The ratio of complications in laparoscopic surgery during the first three years of the study periodis at 2.45% and the percentage in last two years is 1.27%,which provesthe difference is statistically significant(P < 0.05).2.Laparoscopic surgery can be divided intoadnexal surgery,hysteromyoma decollement,total hysterectomy,subtotal hysterectomy,and the malignant tumor group.The difference between the surgical complications of adnexal surgeryand the other four groups is statistically significant(P < 0.05).Moreover,compared with adnexal surgery,hysteromyoma decollement and total hysterectomy,the difference of the malignant tumor groupis statistically significant(P<0.05).3.There is no statistically significant difference(P > 0.05),when comparing surgeons with less than 5 years seniority and surgeons who have 5-10 years of experience.However,considering less than 5 years surgeon seniority and more 10 years,the result is opposite(P < 0.05).Besides,the difference between surgeons possessing 5-10 years of seniority and surgeons having over 10 years is statistically significant(P < 0.05).4.In the consideration of comparison between complicationwith history of abdominal surgery and that without history,the difference is statistically significant(P < 0.05).5.The 85 cases of complications in the sample consist of 10 cases of puncture bleeding,8 with subcutaneous emphysema,7 ureteral injuries,3injuries of bladder,intestinal canal injury in 3cases,8 with vaginal stump hemorrhage,42 cases of postoperative shoulder pain,abdominal planting in 2 cases and 2 with intraoperative great vascular injury.Conclusion:1.The complication incidence of gynecological laparoscopic surgery in Huangshan People’s Hospital from January 2013 to December 2017 was 1.98%,which was slightly higher than the literature report.The proportion of complications of gynecological laparoscopic surgery in research duration is descended in general.2.The complications of laparoscopic surgery are closely related to the operative level and difficulty.The higher the operation level,the higher is the incidence of complications,and vice versa.3.The incidence of complications of laparoscopic surgery is associated with the qualification of surgeons.4.The incidence of laparoscopic surgical complications with a history of abdominal surgery is increased.5.Preventive measures for complications of laparoscopic surgery: To master the characteristics of laparoscopic complications,to take preventive measures,to timely detect and deal with complications during laparoscopic surgery,and to minimize the damage of complications.
