Objective:To study the efficacy and effect of atomization inhalation recombinant human interferon α-2b on lymphocyte in treatment of children with bronchiolitis.Try to provide further theoretical basis for the clinical mmunotherapy in the future.Methods: Select 40 cases of young children with Respiratory Syncytial Virus(RSV)bronchiolitis in the First Affiliated Hospital of Wannan Medical College who aged from 7 to 24 months in one and half years and 15 cases of young children age from 7 to 24 months as normal group,children with bronchiolitis were randomly divided into two groups.The control group,therapy group and normal group showed little difference in body weight,age and gender(P>0.05).Respiratory syncytial virus Ig M antibody was positive in RSV bronchiolitis group,and the diagnosis of RSV bronchiolitis was according to Zhu Futang Practice of Pediatrics 8th Edition.There were no use other antiviral drugs and immunomodulatory agents in 2 weeks and no other severe complications in these children with bronchiolitis.There are no past history of allergic disease and anaphylactic family history in normal group which were healthy children.The control group had routine and symptomatic treatment,therapy group inhaled recombinant human interferon α-2b 200000IU/kg.The basic treatment included Salbutamol Sulfate Solution for Inhalation,Budesonide Suspension for Inhalation.Children with RSV bronchiolitis received 2.0-2.5 ml of fasting peripheral venous blood within 24 hours after hospitalization and 1 week after treatment.The infants in the normal group received 2.0-2.5 ml of fasting peripheral venous blood for fasting during physical examination.The percentage of lymphocyte subsets were analyzed by flow cytometry.We used four-level-scoring method to evaluate disease contidions.Results: 1.The percentage of lymphocyte subsets(1)CD3+ T cells :The brochiolits group decreased significantly compared with the normal group(P <0.01);and increased after treatment(P <0.05),but the therapy group increased compared with the control group(P <0.05).(2)There were little significant discrepancies in CD3+CD4+ T cells among the three groups(P > 0.05)(3)CD3+CD8+ T cells :The brochiolits group decreased significantly compared with the normal group(P <0.01);and increased after treatment(P <0.05),but the therapy group increased compared with the control group(P <0.05).(4)The ratio of CD4+/CD8+ :The brochiolits group increased significantly compared with the normal group(P <0.01);and decreased significantly after treatment in therapy group(P <0.01),but the therapy group decreased compared with the control group(P<0.05).(5)CD3-CD16+56+ NK cells :The brochiolits group decreased significantly compared with the normal group(P < 0.01);and increased significantly after treatment(P <0.01),but the therapy group increased compared with the control group(P <0.05).(6)CD3-CD19+ B cells :The brochiolits group increased significantly compared with the normal group(P < 0.01);and decreased significantly after treatment(P <0.01),but there were little significant discrepancies in CD3-CD19+ B cells among the therapy group and control group(P >0.05).2.Clinical efficacy observation(1)The total improvement rates of therpy group were higher than control group(P<0.05).(2)Therpy group had obviously improved primary index(wheezing,wheezing rale and three depression sign)compared with control group(P <0.05).(3)The hospital day of therapy group were shorter compared with those of the normal group(P <0.05).(4)Children in therpy group did not show irritation symptoms in local respiratory tracts,and no serious adverse reaction was observed.Conclusion: 1.Immune dysfunction is involved in the process of RSV bronchiolitis,which mainly manifests numerical abnormality and ratio imbalance of lymphocyte subsets.The lymphocyte subsets in children can gradually return to normal after treatment.The treatment for RSV bronchiolitis in young children by nebulized recombinant human interferon α-2b allows children to recover dramatically on CD3+ T cells,CD3+CD8+ T cells,the ratio of CD4+/CD8+,CD3-CD16+56+ NK cells that can significantly regulate children with RSV bronchiolitis in peripheral blood lymphocyte subsets balance and enhance immunity.2.Clinical Observations on the treatment of atomization inhalation recombinant human interferon α-2b on RSV bronchiolitis shorter hospital stay.
