Clinical Observation Of Yiqi Yangyin Tongluo Combined With Glucocorticoids In The Treatment Of Cryptogenic Organizing Pneumonia

Objective:To observe the clinical curative effect of treating cryptogenic organizing pneumonia(COP)with the Combination of yiqi yangyin tongluo fang and glucocorticoid.Method:A total of 15 COP patients were collected.The syndrome of TCM syndrome was Qi yin deficiency and phlegm stasis syndrome.All patients were treated with the Combination of yiqi yangyin tongluo fang and glucocorticoid.The course was 6 months and followed up for 1 year to see if there was a recurrence.To evaluate the clinical curative effect by statistic and compare the comprehensive curative effect,TCM symptom score,lung function(DLco,VC,FEV1),chest HRCT,CAT questionnaire,recurrence rate and safety index.The results were obtained by statistical analysis using SPSS 20.0.Results:1.After 6 months of oral treatment,8 cases were controlled clinically,3 cases were effective,3 cases were effective,1 case was invalid,the total efficiency reached 93.3%.The combination of yiqi yangyin tongluo fang and glucocorticoid plays a good role in improving the clinical symptoms and alleviating the disease in COP patients.2.Before treatment,the score of TCM syndromes was 18.80 ± 5.94,and after 6 months of oral therapy,the score of TCM syndromes was 6.13 ± 3.74,P<0.05.Of the 15 patients,9 were effective,5 were effective,1 was invalid,and the total efficiency reached 93.3%.Cough symptoms scores and Shortness of breath scores are decreased after the treatment(P<0.05),which showed that the combination of yiqi yangyin tongluo fang and glucocorticoid could significantly improve the cough and shortness of COP patients.3.After 6 months of treatment,the patients’ HRCT improved,9 out of 15 patients with the disease fully absorbed,6 patients with the disease area reduced more than 1/2.The results showed that the combination of yiqi yangyin tongluo fang and glucocorticoid therapy had positive effect on imaging changes.4.After 6 months of treatment,there was a significant improvement in DLCO and VC compared with that before treatment(P<0.05),but no significant difference in FEV1(P>0.05).The results showed that the combination of yiqi yangyin tongluo fang and glucocorticoid could improve the tidal volume and dispersion of COP patients.the glucocorticoid therapy.We hypothesized that the reason may be that Chinese medicine and glucocorticoid could improve the tidal volume and dispersion of COP patients.5.CAT score was 16.80±4.96 before treatment,and after 6 months,patients with CAT score was 5.20±2.36.The results showed that the combination of yiqi yangyin tongluo fang and glucocorticoid was beneficial to improve the quality of life of patients.6.The total course of glucocorticoid therapy increased with the increase of course of treatment.The results showed that the earlier the intervention,the shorter glucocorticoid cooperate to alleviate the disease,on the other hand,Chinese medicine can antagonize the side effects of glucocorticoid therapy.As a result,patients benefit more from the addition and subtraction of glucocorticoid to treat COP than they do with glucocorticoid alone.7.At the end of the experiment,the patients were followed up for 1 year,with 1 recurrence in 15 patients and a recurrence rate of 6.7%.The recurrent rate of COP in this study was lower than the recurrent rate reported in the literature.And it is indicated that the combination of yiqi yangyin tongluo fang and glucocorticoid could reduce the recurrent rate of COP.8.In the whole course of clinical observation,all patients did not have serious adverse reaction,which showed that the combination of yiqi yangyin tongluo fang and glucocorticoid had good safety in COP treatment.Conclusion:The results of this clinical study show that the treatment of cryptogenic organizing pneumonia by the yiqi yangyin tongluo fang and glucocorticoid can improve the clinical symptoms,improve the quality of life of patients,reduce the clinical recurrence rate,there is a certain clinical effect,and yiqi yangyin tongluo fang has good safety in the treatment.The results of this study provide objective clinical data for the treatment of COP in combination with traditional Chinese and Western medicine,and also compensate for the lack of long-term clinical follow-up,as well as reference for modern medical recognition and treatment of COP.
