| Objective:This research is to explore and analyze the characteristics and disciplinarians of TCM syndromes and prescriptions of Chinese Medicine of patients with permenopausal anxiety and depressive disorder using methods of data mining with the help of literature research and clinical observation.Research methods:Literature research:The study collected literatures about permenopausal anxiety and depressive disorder from CNKI,then established database and used statistical methods for data processing,and used TCM theory to analysis statistical results finally.Clinical observation:The study collected clinical date of 30 patients with permenopausal anxiety and depressive disorder about general date,HAMA,HAMD,Kupperman,TCM syndromes and prescriptions of Chinese Medicine from June 2017 to December 2017 in my tutor’s Encephalopathy clinic of Dongfang hospital of BUCM.Statistical methods:The data was collected and input,analysed using SPSS 20.0 and SPSS modelerl8.0,and then talk about the main symptoms and TCM prescriptions by the statistical methods of association rules and cluster analysis.Results:Literature research:(1)General situation:There are 108 clinical researches having met the requirements with 90 study designs using random method(83%),85 articles using comparative studys(79%),and 0 article were blinded.Short course of treatment is 2 weeks,long for 1 years and most for 8-12 weeks.The total number of tests change from 20 to 236,and the number of test groups change from 20 to 150;the research objects for perimenopausal depression are 96,menopausal anxiety 9,perimenopausal depression and anxiety 3;test group and Western medicine control subjects of 49 articles,49 articles and Western medicine combining Chinese and Western medicine,Chinese medicine and other 5 were divided into three groups.Control group interventions are antidepressants,antidepressants combined with estrogen and estrogen.(2)Characteristics of syndrome:There are 25 commen type of syndrome:kidney deficiency and liver stagnation(34%),liver qi stagnation and spleen deficiency syndromespleen deficiency(7%),liver qi stagnation(7%),yin deficiency of liver and kidney(6%).(3)Disciplinarians of TCM prescriptions:There are a total of 169 kinds of Chinese medicine are used,and the single Chinese herbs with frequency greater than 10 times are sorted.There are 15 kinds of sorting by the former.According to the classification of drug efficacy,the use of supplementing deficiency drugs was the most(26%),followed by anantic drugs(9%),blood activating medicine(9%),antipyretic drugs(8%),Qi medicine(8%),liver extinguishing drug(7%),and antipyretic drugs(6%).There are 108 prescriptions.The high frequency of prescriptions are Xiaoyaosan and Suanzaoren Decoction,ganmaidazao decoction,entrapement keel,Oyster Soup Rehmanniae Decoction of six ingredients.Based on the analysis of association rules,a total of 49 groups of core drug combinations(drug pairs)with high support and high confidence were obtained.The effect was mainly to nourish the kidney,dredge the liver,dredge the liver and invigorate the spleen,nourish the Qi and nourish the blood,carry the Qi and blood circulation,and nourish the heart and the God.Clinical observation:(1)The general situation of the patients:The study collected a total of 30 cases;age group mainly concentrates in 51 to 60 adults;most patients are married;education level is given priority to college graduates or junior college graduates.People with more extroverted personalities are inclined to get this disease;the course of the disease is main focus on 1 to 3 years;symptoms of most patients are body symptoms not mental symptoms.(2)The degree of anxiety,depression and climacteric is mainly mild to moderate.(3)Symptom characteristics:mental depression(83%),dyspepsia(57%),impatience(47%),irritability(43%),fatigue(43%),dizziness(43%),difficulty in falling asleep(43%),excessive thinking(40%),interest(33%),hot flushes and night sweats(33%);Most tongue pictures are pink tongue,thin white fur or dark red tongue and red tongue.Most pulse conditions are deep pulse,thready pulse,taut pulse,rapid puls and slippery pulse(4)Characteristics of TCM syndromes:Four ragular types of syndrome:syndrome of kidney deficiency and stagnation of liver(30%),syndrome of deficiency of both heart and spleen(25%),syndrome of deficiency of both yin and yang of kidney(12%),syndrome of qi depression transforming into fire(10%);syndrome of kidney deficiency and stagnation of liver,syndrome of deficiency of both heart and spleen,syndrome of qi depression transforming into fire and syndrome of intermin-gled phlegm and blood stasis are the four regular syndrome based on methods of system clustering.(5)Disciplinarians of TCM prescriptions:115 kinds of herbs are used commenly like Licorice,angelica,bupleurum,Radix Aucklandiae,Cortex Moutan,Suanzaoren,Poria,Poria,Radix Codonopsis,gardenia.According to the classification of drug efficacy,the use of supplementing deficiency drugs was the most(26%),followed by antipyretic drugs(11%),antipyretic drugs(10%),Qi medicine(10%),antipyretic drugs(9%),blood activating medicine(8%)and phlegm medicine(7%).Based on the analysis of association rules,55 groups of core drugs(drug pairs)with high support and high reliability were obtained.The efficacy of the drug was to complement kidney,liver,spleen,blood,Qi,blood circulation and Anshen.Conclusion:Literature research(1)Most of the clinical trials in the literature are randomized controlled trials,and there are no blind researches.Traditional Chinese medicine comparing with Western medicine and combination of traditional Chinese and western medicine comparing with Western medicine are the main way to compare.Treatments with simple antidepressant drugs are much more than combined antidepressant and estrogen.Most course of the treatment is 8 to 12 weeks.Permenopausal depressive disorder is the main subjects.(2)Syndrome of kidney deficiency and stagnation of liver is the main syndrome type and we should get to know the pathogenesis of perimenopausal anxiety and depression from the aspect of Zang and Fuviscera,Qi and Blood,meridian and other angles,especially the application of the theory of "renal brain correlation".(3)The use of Bupleurum is the highest,followed by Angelica,Paeonia lactiflora,tuckahoe,radix rehmanniae,sour jujube kernel,tulip,Fructus Corni,etc.We should attach importance to the application of tonifying and drugs for regulating QI and activating blood.(4)Tonifying the kidney and dispersing the liver is the main treatment method,combining the treatment methods of nourishing blood,activating blood,invigorating the spleen,nourishing the heart,and so on.Rehmanniae Decoction of Six Ingredients and Water-Enriching Liver-Clearing Beverage are very important.Clinical observation:(1)Most patients are married,education level is over the middle level,and age group mainly concentrated at tlhe age of 50 to 60.Most of the patients have physical symptoms,and insomnia is a physical symptoms that can not be ignored.The severity is mild to moderate.(2)Syndrome of kidney deficiency and stagnation of liver is the main syndrome type.The core pathogenesis of female perimenopausal anxiety and depression can be summed up as kidney deficiency and liver stagnation.Besides,the application of Chinese and western theory of colonization axis in the occurrence and treatment of disease should be paid attention to.(3)The use of Radix Angelicae,is the highest,followed by Radix Bupleuri,wood aroma,peel,Ziziphus jujube kernel,Poria cocos,Poria,gardenia,Radix Salviae,Radix Paeoniae,raw oysters,raw dragon bone,Ligusticum chuanxiong and so on.The application of tonifying medicine,antipyretic medicine,antipyretic medicine and qi activating and blood activating medicine should be paid attention to.(4)Clinically,the tutor often treats anxiety and depression with Dan Zhi Xiaoyao Powder,and Chaihu plus dragon bone oyster soup.For perimenopausal patients,the tutor always adds raw land,Cornus officinalis,cynomorium,kimoni and other drugs to nourish the kidney yin and warm the kidney. |
