| Objective:Gynecological laparoscopic surgery is widely used in the field of gynecologic surgery,which has more advantages than laparotomy,such as smaller wounds,less bleeding,shorter length of hospital stay,and so on.However,considered some inevitable procedures of surgery,like anesthetization,CO2 pneumoperitoneum and head-low-food-high position t,laparoscopic surgery also brings some complications and uncomfortable feelings to patients during the postoperative time.Gastrointestinal motility and function disorder is the most common one.Clinically,this condition ranges from postoperative nausea,vomiting,absent bowel sounds to prolonged time of flatus,and abdominal distention,even add the possibility of postoperative ileus.So,it will have a great significance for gynecological and pelvic surgery,to find strategies hastening gastrointestinal recovery after surgery.This study is going to assess whether Houpo Paiqi Mixture accompanied with chewing gum accelerates the recovery of gastrointestinal function.Methods:Patients:The study was conducted from July to November,2016.It was performed on patients who underwent laparoscopic surgery in department of gynecology in the East Hospital,the second affiliated hospital of Beijing University of Chinese Medicine.The patients were divided randomly into the test group and the control group,which included 38 cases and 37 cases respectively.Of the 75 patients,one was excluded after assignment to the text group because she was found to have endometrial cancer after her laparoscopic surgery.Materials:Extra xylitol gum of cool mint(William Wrigley Jr Company,Shanghai,IL,PRC);Houpo Paiqi Mixture(Reyoung Pharmaceutical Co.,Ltd.).Groups:The test group received a treatment,which had two steps.First,postoperative patients were instructed to drink 10ml Houpo Paiqi Mixture 6 hours later after surgery,then,chewing 2 gums for 15minutes,those two steps repeated every 4 hours(from 7:00 a.m.to 10:00 p.m.),until they defecated.The control group had no intervening measures.Both two groups were observed 72 hours,in the meanwhile,they all received routine care and treatment after surgery.Main observational index:All cases need to be recorded their time of first normal bowel sounds,time of first flatus,time of first defecated,the level of abdominal distention and pain after surgery and 72 hours later,and the length of hospital stay.Data analysis:Excel 2016 is used to record basic data.SPSS 21.0,the statistical software package,is used for data calculation and analysis.T-test is adopted when measurement data is of normal distribution and variance together,while nonparametric test is adopted with abnormal distribution.The basic chi-square test or Fisher’s exact test is adopted with Count data R*C.The value of P equal to or less than 0.05 is considered to be of statistical significance.Result:In this study,there are no significant differences between patients of two groups in aspects of ages,ways of surgery,surgeons,duration of surgery,degree of pelvic adhesions,intra-operative blood loss,analgesia pump,antibiotics and hemostatic,which are comparable(P>0.05).1.The first time of normal bowel sounds,time of first flatus,time of first defecated in the test group(14.39±4.150h,15.53±4.482h,32.30±14.906h)is in advance compared to those of the control group(24.14±8.389h,27.46±9.419h,45.33±14.454h).(P<0.05).2.The degree of abdominal distention and pain of patients postoperatively in the test group is lighter than those of the control group.(P<0.05).The morbidity of nausea and vomiting of the test group(2.7%)is lower than the control group(10.8%).3.The length of hospital stay after surgery in the test group(6.81±1.729d)is shorter than in the control group(7.57±3.141d).(P<0.05).Conclusion:Chewing gum accompanied with Houpo Paiqi Mixture can decrease time to first normal bowel sounds,first flatus and first defecated,release the level of abdominal distentiond and the degree of abdominal pain,lower the morbidity of nausea and vomiting during the postoperative time,and cut down the length of hospital stay.So it is demonstrated to promote the recovery of gastrointestinal function. |
