Clinical Investigation And Analysis Of TCM Syndromes And Physical Factors Related To Mixed Anxiety And Depression

Objctive:This research is to explore and analyze the characteristics of TCM syndromes,and physical characteristics of patients with Mixed Anxiety and Depressive Disorder(MADD),in order to master the common syndromes and susceptible constitutions.For individualized treatment and improving the level of clinical treatment,we further explore and analyze the correlation of TCM syndromes and constitutions of MADD,appling the diagnosis and treatment mode of "Distinguish diseases-Distinguish constitutions-Distinguish syndromes" to clinic effectively.Methods:This research with the support of Beijing University of Chinese Medicine(BUCM)Postgraduate Students Independent Subject.We collected 61 eligible patients of MADD in Encephalopathy clinic of Dongfang hospital of BUCM.Meanwhile,30 healthy persons were selected as the research objects in August 2016 to January 2017.They filled in TCM constitutions classification and determination scale,compared to the similarities and differences of constitutions between them.At the same time,the clinical data of the patients were collected and analyzed to explore the relationship between TCM Constitutions and syndromes.Statistical methods:Use the Excel table to create a database and use SPSS20.0 to process the analysis.General information using descriptive analysis,counting data using chi-square test,grade data taken nonparametric test,all p<0.05 as a statistically significant standard.Combined with the clinical information collected by the four clinics,the use of system cluster analysis to extract the elements of syndromes,summed up the syndrome elements distribution,combination,evolution characteristics.Results:(1)The general situation of MADD:(1)The study collected 61 cases of cases,including women more than men;age mainly concentrated in young adults;education level to undergraduate or college-based;marital status to married mainly;course mainly concentrated in 1 year Within the patient’s anxiety and depression and gender,age,no significant difference.(2)Distribution characteristics of TCM syndromes:61 cases of MADD patients with the distribution of TCM syndromes from high to low.followed by phlegm stagnation syndrome,liver qi stagnation syndrome,qi stagnation of fire.heart and spleen deficiency syndrome.heart and kidney deficiency syndrome and lack of heart,which Phlegm stagnation syndrome as the main type.According to the clinical symptoms and signs of patients and the use of systematic clustering analysis method,MADD disease syndrome mostly qi stagnation,phlegm,blood stasis,yin deficiency and yang deficiency,combined with tutor clinical experience and related research progress Classified as three categories:were sputum gas stagnation syndrome,gas stagnation and blood stasis syndrome and yin and yang deficiency syndrome.In addition,MADD syndromes are not affected by age,duration and anxiety and depression.(3)Distribution characteristics of TCM physique:61 cases of MADD patients with the most qi stagnation,followed by qi deficiency,yang deficiency and damp heat,yin deficiency,phlegm and blood stasis in the disease rare;30 cases of healthy people in the control group to the highest quality,Followed by yang deficiency and hot and humid more common,both of which are yang deficiency and damp heat,but MADD patients with qi stagnation performance mainly qi deficiency,while the control group to the highest quality.At the same time,the survey suggested that 61 cases of MADD patients with yin deficiency,phlegm,wet and heat,blood stasis to anxiety-based,yang deficiency,qi stagnation to depression-based performance,qi deficiency to anxiety and depression performance differences Equal to the main.In addition,according to the survey,MADD patients with Chinese medicine constitution and age growth and the length of the course is no significant difference,suggesting that the individual physical distribution is not easy to change,its inherent characteristics are relatively constant.(4)The correlation betw een TCM syndromes and physical:61 cases of MADD patients,liver qi stagnation syndrome in the qi stagnation(46.2%)the most;qi stagnation of qi stagnation(36.4%)more common;phlegm stagnation syndrome phlegm and qi stagnation more common(23.8%);Heart and spleen deficiency in the qi deficiency(44.4%)the most:lack of health for the blood stasis up to(100.0%):heart and kidney yin deficiency syndrome in the most(50.0%).At the same time,qi stagnation(18)in the highest frequency of liver qi stagnation syndrome(6),qi deficiency(9)central spleen deficiency test the highest frequency(4),Yang deficiency(9)in the sputum(4)were the highest frequency,and the highest incidence of liver qi stagnation syndrome and qi stagnation syndrome(3 persons)was the highest in the hot and humid quality(9 persons).The yin deficiency syndrome(7 persons)(2),the highest frequency of phlegm(6).The highest frequency of phlegm and blood stagnation syndrome(2)was the highest(2).Conclusion:(1)MADD patients were mostly female,married and educated at the middle level,while the low level of education and low,and MADD patients were younger age of onset of the trend.(2)Qi stagnation,phlegm,blood stasis,yin deficiency and yang deficiency are closely related to the pathogenesis of MADD.It can be seen,MADD TCM syndromes are complex and diverse,syndromes of the actual type of mixed,leading to recurrent disease,lingering refractory.(3)MADD sick group compared with the normal group,the prevalence of mostly qi stagnation of the population,while the normal population mostly flat quality,both yang deficiency and damp heat.Yin deficiency,phlegm,wet and heat,blood stasis to the main performance of anxiety,yang deficiency,qi stagnation to the main performance of depression,qi deficiency to anxiety and depression performance differences are the main difference.Therefore,the clinical identification of disease and the combination of identification,timely correction or improvement of partial quality,can reduce the individual’s physical susceptibility to the disease,so that disease prevention,both disease prevention.(4)To explore the incidence of MADD with syndrome as a factor,it has obvious correlation with the patient’s own constitution.When the syndrome of MADD is used as the element,the two are not exactly the same,but there is still some correlation.It can be seen,the clinical should still be based on dialectical,combined with disease identification and identification in order to cure the disease in the process to play a better effect.
