"If there is no national health, there weren't being an overall well-off society",medical and health service is related to the livelihood of the people and the health of hundreds of millions of people. However,in recent years, the credible doctor-patient relationship which is formed by the “the pattern of difference sequence” in traditional rural China is suffered continually. The frequent medical disputes exacerbate the problems of the doctor-patient relationship. There are significant differences of cognition between doctors and the public in relevant issues. This paper focused on the topic of "Cognitive Differences between Doctor and Patient in Medical Disputes",and made a deep analysis of 40 major medical disputes during 2014 to 2016,and described the situation and characteristics of medical dispute in the network environment. At the same time, through the analysis of media text and the discourse analysis of doctors and patients,we found that there were significant cognitive differences between doctors and patients, and then analyzed the causes of that differences. The study found that the significant cognitive differences between doctors and patients mainly exited in 5 parts, which included "the expectation of therapeutic effects, the judgment of therapeutic methods, the evaluation of therapeutic process, the inference of therapeutic behaviors and the attribution of therapeutic results".The leading factors of the formation of cognitive differences between doctors and patients mainly included the role of doctors and patients, knowledge, contacts, emotions, attributions and so on.The cognitive bias in social identities of doctors and patients,in essence, leads to differences in cognition between doctors and patients. The conflicts in the knowledge structure between doctors and patients, the conflict between "direct" and "indirect" communication model are the root cause; The poor communication impedes the effective interaction between doctors and patients, thus lacks the humanistic communication to the patients;Besides,long-term negative emotional accumulation may directly affects the rational judgment of doctors and patients; Finally, the attribution bias, which attributes mistakes mainly to others, directly affects the cognition of doctors and patients and leads to disputes and conflicts. On the above basis, this paper put forward some suggestions on how to reduce the cognition differences between doctors and patients and construct the harmonious doctor-patient relationship from the aspects of health care reform,doctor-patient communication, health communication, medical popularization and legal protection.
