Objective: At present, the worldwide incidence of lung cancer and its fatality rates are increasing year by year.In this paper,we mainly analyzed the related factors of recurrence and metastasis of lung adenocarcinoma which accounted for the largest proportion of non-small cell lung carcinoma(NSCLC)histopathological subtypes, and then explore the vessel invasion(VI)in patients with early stage lung adenocarcinoma adverse prognosis. Methods: Patients who were diagnosed with lung adenocarcinoma after admission to our hospital from January 2011 to November 2015 and directly treated with "standard single lobectomy and systemic lymph node dissection" were collected through a systematic review. Before and after surgery without any anti-tumor-related treatment, samples were collected to meet the postoperative pathological findings of lung adenocarcinoma, systematic dissection of lymph nodes and the rest of the body before surgery did not occur metastasis, postoperative pathological findings related reports of vessel invasion and other related conditions. After collecting 78 cases of relevant cases meeting various screening criteria, single factor statistical analysis was performed on the recurrence and metastasis of cancer and its related factors within 2 years after operation. The t-test was performed for age.Pathological staging, gender, smoking history, location of surgical removal of the lung lobes, tumor thrombus were tested by chi-square test.Statistic test level α=0.05,when P<0.05,it indicates that the statistical difference is significant.Use SPSS19.0 software for statistical analysis.Results:(1)The incidence of recurrence and metastasis in patients with early stage lung adenocarcinoma within 2 years after operation was not statistically significant in terms of age, pathological stage, sex, smoking history, surgical resection of lung lobes(P>0.050).(2)In the early stage of pulmonary adenocarcinoma, the rate of recurrence and metastasis of the positive group of vessel invasion was 36.59% within 2 years after operation, while the negative group of vessel invasion only 16.22%.Two groups of patients within 2 years after the occurrence of recurrence and metastasis differences were statistically significant(P =0.043).Conclusion: There is a correlation between the presence of vessel invasion in patients with early stage lung adenocarcinoma and the recurrence and metastasis within 2 years after operation. And patients with vessel invasion were found to have a higher probability of recurrence and metastasis of the tumor within 2 years after operation. Therefore, the vessel invasion can be used as an unfavorable prognostic factor in patients with early stage lung adenocarcinoma.Meanwhile,we suggest that in clinical practice, based on the TNM staging, the vessel invasion should be included in the early stage lung adenocarcinoma patients with short-term(2 years)recurrence and metastasis in the evaluation of indicators. |
