Objective:To determine the expression of typeⅠand typeⅢcollagen contents in transversalis fascia of adult indirect and direct inguinal hernia, and compared with non-hernia patients. Methods:The subjects were obtained from patients who accept surgical operation from December 2016 to December 2017 in The Affiliated Hospital of Qinghai University, all patients are male. Each trial group include 20 cases primary indirect inguinal hernia(IDH)and direct inguinal hernia(DH).The control group was 20 cases non-hernia(NH)patients who accept lower abdominal surgery.Exclude non-research factors that may affect abnormal collagen metabolism.Tissues specimens were resected far from the hernia sac relatively normal medial transversalis fascia, the area of the organization was about 0.7*1.0cm~2.The contents of typeⅠandⅢcollagen were detected using immunohistochemical methods and the system of computer image collection and analysis.Results:(1)There was no significant difference in age among groups(P>0.05).(2)TypeⅠcollagen content(IOD):the control group was 153.32±10.59,the indirect inguinal hernia group was 123.71±8.20,the direct group was102.31±7.94,the indirect inguinal hernia group and direct hernia group all showed lower expression than the control group(P<0.05);the direct group showed lower expression than the indirect group(P<0.05).(3)TypeⅢcollagen content(IOD):the control group was 103.33±5.33,the indirect inguinal hernia group was 119.71±7.28,the direct group was 131.10±8.47,the indirect inguinal hernia group and direct hernia group all showed high expression than the control group(P<0.05);the direct group showed high expression than the indirect group(P<0.05).(4)The TypeⅠandⅢcollagen ratio:the control group was 1.49±0.10,the indirect inguinal hernia group was 1.04±0.09,the direct group was 0.78±0.06,the indirect inguinal hernia group and direct hernia group all showed lower expression than the control group(P<0.05);the direct group showed lower expression than the indirect group(P<0.05).Conclusion:The structure and metabolism of typeⅠandⅢcollagen molecules in the transversalis fascia of adult primary inguinal hernia was changed and the cross-linking decreased. The balance between synthesis and catabolism is broken, metabolic disorder of collagen in the transversalis fascia, leading to a decrease in its intensity,it may be an important factor in the occurrence and development of primary inguinal hernia. And the metabolic disorder of collagen may play a more important role in the occurrence and development of direct inguinal hernia. |
