Objective: With the difference of the quantitative parameter for dynamic contrast enhanced magnetic resonance imaging(DCE-MRI)in the marginal zones of different types of Hepatic alveolar echinococcosis(HAE)explored,we can analyze both microcirculation changes appearing on the marginal zones of HAE and morphological changes and assess the potential application value in the diagnosis and treatment of HAE.Materials and Methods: A 1.5TMRI dynamic enhanced scan was performed on 30 patients who was diagnosed as HAE in accordance with the clinical observation,laboratory examinations and image.And on the basis of the composition of the lesion,these 30 patients into two groups by the associate doctor taking part in the assessment.The quantitative parameters for dynamic contrast enhanced magnetic resonance imaging of the marginal zone for HAE which was measured by means of the Hemodynamics software are as follows: capacity transfer constant(Ktrans),rate constant(Kep),and the percentage of extracel lular intercellular space volum(Ve).These constants will be analyzed statistically by the software named SPSS 19.0.Results:Different patients had different lesions.Of 30 patients,19 patients(63.3%)were diagnosed as their lesions were all located in the right lobe of the liver,8 patients(26.7%)were examined that the accumulation of lesions in both the left lobe and the right lobe of liver,26 patients(86.7%)faced the situation that the lesions endangered their hepatic vein or portal vein,and 24 patients(80.0%)confronted that the lesions occurred in intrahepatic bile duct.The maximum transverse diameter of the lesion ranged from 5.68 to 12.26 cm.Here are different shapes of the lesions,mainly elliptic and irregular.At the same time,Of the 30 cases,18 cases(60%)were necrotic,and the real type(40%).5 patients confronted that the right adrenal gland was infringed,9 patients were diagnosed that there were multiple enlarged lymph nodes in the hepatic and gastric Spaces and retroperitoneum.Besides,we found that the lesions of 4 patients spreading to other parts of the body.(There were two cases of double lung metastases,one cases of disseminated metastases in the brain and one cases of vertebral metastases,and all of them were diagnosed by the deputy chief physician).Ktrans solid group edge(0.178±0.292),Kep(0.488±0.063)than the surrounding liver tissue(Ktrans,Kep and Ve respectively(0.115±0.074)/min,(0.352±0.179)/min,0.328±0.022),the difference was statistically significant(p<0.01),solid edge group with Ve value(0.369±0.064)compared with the surrounding liver tissue,the difference was not statistically significant(p>0.05),Ktrans,Kep and necrosis group numerical parameters(Ve(0.395 ±0.285)/min,(0.784±0.304)/min,0.504±0.436)were higher than the surrounding liver tissue((0.129±0.066)/min,(0.382±0.134)+ /min,0.356±0.098),the difference was statistically significant(P<0.01);group Ktrans,Kep with edge necrosis and the parameters of Ve value((0.395±0.285)/min,(0.784±0.304)/min,0.504±0.436)was significantly higher than that of the solid group((0.178±0.2 92)/min,(0.488±0.063)/min,0.369±0.064,and the difference was statistically significant(p<0.01).Conclusion: In terms of morphology,DCE-MRI can reflect the distribution and invasion characteristics of lesions for HAE.From a microscopic perspective,the quantitative parameter can reflects the difference of the change of capillaries for different types of HAE edge infiltration,which provides a certain value for the clinical assessment to the value-added activity of marginal zone of HAE and different applicable ways of operation.
