Objective: To compare therapeutic effects of selective neck dissection(SND)and comprehensive neck dissection(CND)to patients with oral squamous cell carcinoma(OSCC)(cN0、pN+).Methods: To collect patients with a diagnosis of OSCC(cN0、pN+)of tongue or mouth floor from General Hospital of Ningxia Medical University from January 2013 to December 2016.These patients were divided into the SND group and the CND group based on different dissection methods of cervical lymph node,and information of age,gender,cTNM,pTNM,pathological stage,smoking history,alcohol history,lymph node status were recorded.Timing started after the first treatment,and physical exams were performed for three years: once every three months for the first two years and once every six months for the last year.During each examination,lymph node status were checked by cervical CT examination,and neck configuration,sensation,mobility and shrug were evaluated.Recurrence was judged based on neck lymph node palpation: neck lymph node being not touched and lumps and ulcers being absent in the primary lesion were regarded as recurrence-free;neck lymph node being touched or lumps and ulcers being present in the primary lesion were regarded as recurrence.Statistical analysis was performed after collecting recurrence situation after the treatment of above methods.x2 test was adopted to compare baseline information.P value < 0.05 was considered as statistically significant.Results: We collected 119 patients with a diagnosis of OSCC(cN0、pN+)of tongue or mouth floor: 56 patients in the SND group and 63 patients in the CND group.No significantdifference of the baseline data of two groups were found(P>0.05).There were no significant difference in 3 years neck control rate(25.0% vs 28.6%,P>0.05)between theses two groups.Patients in SND group had fewer complications than in CND group(3.6% vs 12.7%,P<0.05).Conclusions: SND was better than CND for patients with a diagnosis of OSCC(cN0、pN+)of tongue or mouth floor. |
