Objective:This paper aims to investigate the correlation between serum homocysteine level and recurrent cerebral infarction and carotid atherosclerosis among the people in Xining area;It aims to evaluate the clinical guidance and significance of the homocysteine and cervical vascular Doppler color ultrasonography in the cerebral infarction recurrence in Xining area and provides the theoretical basis for preventing the recurrence of cerebral infarction.Methods:Being screened from the stroke screening table in the Xining area,patients with cerebral infarction two times or more were selected as the recurrent cerebral infarction group,and those with cerebral infarction one time were selected as the initial group,the healthy group who was examined in our hospital during the same period were selected as the control group.The demographic characteristics such as gender,age,ethnicity,education level,occupation,etc.,as well as past medical history,drinking history,and smoking history were all screened and recorded in order.It was recorded whether the patients have the hypertension,diabetes,coronary heart disease,high homocysteine,hyperlipidemia or not.Based on the cranial MRI and carotid artery ultrasonography results,the carotid artery stenosis,plaque formation,and the presence or absence of the internal carotid artery thickening were recorded,and the serum HCY and serum lipids were also measured.Results:(1)Compare the relative factors between recurrent cerebral infarction group and initial cerebral infarction group,through the statistical analysis,the age,gender,nationality,occupation,and education level difference between two groups are statistically significant,and the homocysteine,hypertension,and hyperlipidemia difference between two groups were statistically significant.(2)96 patients with recurrent cerebral infarction all have carotid atherosclerotic plaques(100%),among which 82 patients(85.42%)have vulnerable plaques,and 96patients have HHcy(100%).There are 86(83.50%)patients with carotid atherosclerotic plaques in 103 patients with primary cerebral infarction,among which78(75.73%)have vulnerable plaques and 89 patients have HHcy(86.41%).In the control group,there were 13 patients(12.75%)with carotid atherosclerotic plaques,of which 12 patients(11.76%)have vulnerable plaques,and 49 patients(48.04%)had HHcy.The differences between the two groups were statistically significant(P<0.01).(3)The Serum Hcy level in the recurrent group,initial cerebral infarction group,and control group is(38.10±4.74)mmol/L,(18.87±3.63)mmol/L,and(15.21±3.63)mmol/L respectively.The statistical indicates that the erum Hcy levels were significantly different among the three groups(x~2=918.72,P<0.01);(4)The vascular stenosis incidence in the recurrent cerebral infarction group,the initial cerebral infarction group,and the control group is 97.92%(94),73.79%(76),and 13.73%(14),respectively.The vascular stenosis incidence among the groups is statistically significant(P<0.01).(5)The cerebral infarction recurrence group was divided into five groups based on the carotid artery stenosis degree.The serum Hcy level is:no stenosis group(32.74±?2.60)mmol/L,mild stenosis group(36.55±4.56)mmol/L,Mid-stenosis group(37.36±5.23)mmol/L,severe narrow-slice group(39.84±4.04)mmol/L,and complete occlusion group(40.50±4.30)mmol/L.The Hcy level difference among different groups is statically significant(P<0.05).Conclusions:1.The serum Hcy level,carotid atherosclerosis,carotid artery stenosis are clearly related with the cerebral infarction recurrence among the population in Xining area.2.The patients with recurrent cerebral infarction among the population in Xining area also have the carotid stenosis,and the serum homocysteine level is obviously increased.The serum homocysteine level is clearly related with the carotid artery stenosis degree.3.The serum Hcy level of patients with cerebral infarction is high with vulnerable plaque formation,which may increase the risk of cerebral infarction recurrence.The regular carotid duplex ultrasound check and timely definition of Carotid atherosclerotic plaque trait plays the extremely important role in effectively predicting the recurrence of cerebral infarction.
