Objective:To review the general information and clinical data of patients with premenopausal endometrial carcinoma(EC),the clinical pathological and immunohistochemical features of premenopausal ECpatients,to find out the patients who are overlooked because of the untypical symptoms of the clinical symptoms,in order to further improve the early diagnosis and early treatment rate of premenopausal endometrial carcinoma.Methods:The patients who were admitted and diagnosed with EC in First Hospital Affiliated to Lanzhou University from December 2012 to December 2017 were treated with surgical treatment,208 cases of postoperative pathological diagnosis and immunohistochemical analysis were performed,according to the patient’s menopause,88 cases(case group)and 120 cases(control group)were divided into premenopausal group(case group)and postmenopausal group,then the general clinical,pathological and immunohistochemical features of the two groups were compared,it contributes a little to clinical diagnosis and treatment.Results:1.The premenopausal EC patients in our hospital from December to December 2017 accounted for 42%(88/208)of EC.There was no significant difference in the combination of premenopausal and postmenopausal patients with hypertension,diabetes,hysteromyoma and ovarian begin tumors(P > 0.05).Compared with premenopausal group and postmenopausal group,there was significant difference between in vaginal massive bleeding and obesity,infertility and adenomyosis(P<0.05).2.There was no significant difference in tumor markers CA199 and CA724 between premenopausal group and postmenopausal group(P > 0.05).The difference in CA125 was statistically significant(P<0.05).3.Compared with the premenopausal group and the postmenopausal group,the difference was statistically significant(P>0.05)in the patients with IA stage and IB stage.4.Compared with premenopausal group and postmenopausal group,endometrioid adenocarcinoma occupies the most important position in tumor type,and the difference isstatistically significant(P<0.05).5.Compared with premenopausal group and postmenopausal group,the positive rates of ER,PR,PR negative and ER and PR were all statistically different(P<0.05).Conclusion:1.The occurrence of endometrial carcinoma before menopause is closely related to a large number of vaginal bleeding and the clinical symptoms such as obesity,infertility and adenomyosis.2.The tumor stage of the premenopausal group was mostly IA and IB stage.The treatment effect was good and the survival time was long because of the early stage.3.The tumor types in the premenopausal group were almost all endometrioid adenocarcinoma,and most of them were highly differentiated.The therapeutic effect and postoperative recovery were better.4.The positive rates of ER and PR in premenopausal and postmenopausal groups were statistically significant,which provided effective evidence for further treatment. |
