Objective Through the clinical and statistical analysis of the hospitalization expenses of patients with acute ischemic stroke in a three-class-hospital[1-4]of Gansu Province,that is,the hospitalization expenses of patients with cerebral infarction and their different severity and different treatment methods between 2012-2015,the relationship between the internal structure of hospitalization expenses and the ways of paying and their different severity and different treatment methods were discussed.To provide reference data for reducing the economic burden of acute cerebral infarction patients and related medical insurance policies.Methods The method of retrospective investigation was used.A total of 4788 patients with"cerebral infarction,ischemic stroke and acute cerebrovascular disease"diagnosed in the first diagnosis name of the hospital HIS[5-6]registered in the hospital of the hospital of HIS,Gansu Province,were selected from the first diagnosis of the three grade a hospital of Grade A of the hospital of Gansu province.For the remaining 4781 cases,the NIHSS score was[7-13],and the patients who had been screened out for more than 2 weeks were obtained,and 4561 cases of effective cases were obtained.The basic data of the patients,the cost of hospitalization,the medical pay and the different treatment methods and the severity of the NIHSS score were summarized and the related statistical analysis were carried out.Among them:?1?general data of hospitalized patients:age,sex,average days of hospitalization,time of hospitalization,payment of hospitalization expenses,treatment methods and classification of disease severity?through NIHSS score?.?2?the cost of hospitalization:the total cost of hospitalization,the cost of inspection,the cost of laboratory tests,the cost of Medicine?the cost of Western medicine and Chinese Medicine?,the cost of the operation,the bed cost,the cost of treatment,the cost of nursing and other expenses.?3?using IBM SPSS Statistics 22.0[14]to perform statistical nonparametric[15-20]Mann-test WhitneyU test,Kruskal-Walis test and Spearman grade correlation analysis[21-23].Results The median of the hospitalization costs of the 4561 patients was 12601.03 yuan,?1?the main cost of hospitalization,the western medicine fee?47.32%?>inspection fee?16.84%?>the laboratory fee?16.56%?>Chinese medicine fee?15.10%?.The overall trend of hospitalization costs was stable,only the cost of traditional Chinese medicine decreased between 2013-2014,and then remained after the decline.The new level tends to be smooth.The difference of average hospitalization costs in different years was not statistically significant?P=0.149>0.05?.The difference in the cost of hospitalization for different age,sex,payment of hospitalization,treatment and the severity of disease was statistically significant?P<0.05?.There was no difference in medical insurance between urban residents??2=6.700,P=0.082>0.05?,and there was no difference??2=3.513,P=0.319>0.05?for each year??2=3.513,P=0.319>0.05?in each year,and there was no difference between the new rural medical insurance??2=1.814,P=0.606>0.05?in each year,and the other social medical insurance was different in each year.??2=21.88,P=0.000<0.01?.?4?there was a linear correlation between the length of hospital stay and the total direct hospitalization expenses?R=0.618,P<0.001?.Conclusions The cost of hospitalization for 2012-2015 years of ischemic stroke in a three-class-hospital in Gansu was higher than that of the national per capita THE.The difference in the cost of hospitalization for different age groups,gender differences and hospitalization expenses,different treatment methods and the severity of disease were statistically significant?P<0.05?.The number of hospitalization days and hospitalization costs were the linear correlation.The related departments can adjust the internal structure of hospitalization expenses,adjust the coverage rate of medical insurance for major diseases and serious diseases according to the severity of acute cerebral infarction and different treatment methods,so as to reduce the economic burden of patients with cerebral infarction.
