Objective:To investigate the risk factors of video-assisted thoracoscopic lobectomy for conversion to thoracotomy surgery in patients with non-small-cell lung cancer.Methods:Retrospective analysis was conducted for 65 patients diagnosed with non-small-cell lung cancer undergoing video-assisted thoracoscopic lobectomy for conversion to thoracotomy surgery at The Affiliated Hospital of Zunyi Medical University from Jan.2014 to Jun.2017.In the same period,117 patients diagnosed with non-small-cell lung cancer undergoing video-assisted thoracoscopic lobectomy surgery were selected as the control group.Information of patients including age,gender,smoking history,tuberculosis history,tumor type,tumor location,the maximum tumor diameter,Pleural adhesions,Surgical groups were carefully recorded.Then Chi-square test was used for univariate analysis.Binary logistic regression was used for variate analysis.Results:Univariate analysis revealed that there were significant differences between two groups on proportion of age≥65years(χ~2=9.937,P=0.002),male(χ~2=5.515,P=0.019),tuberculosis history(χ~2=20.486,P<0.001),Upper lung tumor(χ~2=26.239,P<0.001),the maximum tumor diameter≥3.50cm(χ~2=9.543,P=0.002),pleural adhesion Level≥4(χ~2=13.411,P<0.001).Mltivariate Logistic regression analysis revealed that tuberculosis history(OR=6.414,95%CI=2.068~19.893),Upper lung tumor(OR=8.990,95%CI=3.046~26.531),pleural adhesion Level≥4(OR=3.818,95%CI=1.654~8.816)were independent risk factors of video-assisted thoracoscopic lobectomy for conversion to thoracotomy surgery.Conclusion:Age≥65 years,male,with tuberculosis history,upper lung tumor,the maximum tumor diameter≥3.50cm,pleural adhesion Level≥4 were risk factors for conversion to thoracotomy surgery.With tuberculosis history,Upper lung tumor and pleural adhesion Level≥4 were independent risk factors of video-assisted thoracoscopic lobectomy for conversion to thoracotomy surgery.
