Objective: To investigate the main pathogenic types of children with lobar pneumonia in this region,the clinical features of lobar pneumonia of children with the related pathogenic types was analyzed,and to offer reference for clinical doctors to use drugs empirically.Methods: Eighty lobar pneumonia children who were diagnosed in First Affiliated Hispital of Medical college of shihezi university from April 2013 to May 2015 were seleted,including 44 cases of male,female 36 cases,the average age was(8.1±3.2)years,and collect the clnical data.Eighty children were all treated by bronchoalveolar lavage(BAL),Bronchoalveolar lavage fluid(BALF)of patients were collected,Mycoplasma pneumoniae(MP),Chlamydia pneumoniae(CP),Legionella pneumophilia(LP),Streptococcus pneumoniae(SP),Klebsiella pneumoniae(KP),Haemophilus influenzae(HI),Adenovirus(ADV),Respiratory syncytial virus(RSV)were detected and analyzed by FQ-PCR in BALF,the clinical features of lobar pneumonia of children with the related pathogenic types was statistically analyzed.Results:1.In 80 cases,the positive is 70 cases,the positive rate is 87.50%.59 cases were MP(the positive rate is 73.75%),2 cases were CP(the positive rate is 2.50%),12 cases were SP(the positive rate is 15.00%),1 case was KP,8 cases were ADV(the positive rate is 10.00%),1 case was RSV,LP and HI were not detected,14 cases were two and more mixed infection.2.The positive rate of MP detection was different in different age groups,the positive rate of 1~3 age group was 36.36%,the positive rate of 4~6 age group was 90.48%,the positive rate of 7~14 age group was 75.00%,the difference was statistically significant(P<0.05),the MP positive rate of 4~6 age group and 7~14 age group was higher than that in 1~3 age group.3.59 cases of MP pneumonia,include 48 single infection and 11 mixed infection,the incidence of complications in the single infection group was 14.58%,the incidence of complications in the mixed infection group was 81.82%,the difference was statistically significant(P<0.05),the incidence of pleural effusion,atelectasis,localized fibrosis in the mixed infection group was higher than that in the single infection group.4.80 cases with lobar pneumonia were treated by antibiotic and bronchoalveolar lavage during hospitalization,until the time of temperature returned to normal is 48 h at least,the cough symptom was basically relieved and the pulmonary rales was disappeared and then discharged,Treatment is 8 to 14 days of 39 cases,15 to 21 days of 41 cases.Conclusions:1.The pathogen of children with lobar pneumonia varied from MP,CP,bacteria and virus,and MP was the most common pathogen in lobar pneumonia,at the same time,SP and ADV also accounted for a considerable proportion in the region.2.The MP positive rate of 4~6 age group and 7~14 age group was higher than that in 1~3age group.3.In children with MP lobar pneumonia,the children of mixed infection is easy to present with pleural effusion,atelectasis,localized fibrosis.It deserves the attention of clinical doctors.
