Objective: To detect the expression of 20 kinds of helper T cell related inflammatory factors in serum of 53 patients with autoimmune liver disease by microarray antibody chip technique and combine with the clinical biochemical data of liver function and the expression level of the autoantibody.To analyze the expression characteristics and differences in inflammatory factors in serum of patients with autoimmune liver disease(AILD),then predict the signal pathway that may be induced by related inflammatory factors and explore the effect of the group action of inflammatory factors of autoimmune liver disease.At the same time,we hope that we can screen the universal change,characteristic and sensitivity index factors related to AILD disease,and can evaluate the development process and prognosis of AILD patient’s biomarker.Methods: From January 2010 to December 2014,53 patients with autoimmune liver disease(AILD)were selected from Sichuan Provincial People’s Hospital and diagnosed as subjects.Among them,14 patients with AIH-PBC overlap syndrome were treated with AIH-PBC OS)39.During the same period,10 healthy volunteers and 14 patients with hepatitis b virus(HBV)were confirmed served as control group.RayBiotech human Th1/Th2/Th17 cell quantitative antibody chip kit(QAH-TH17-1,RayBiotech Inc.USA)was used to measure the serum samples of 53 patients with venous blood samples,and combined with the clinical data of liver function and autoantibody expression level.The population characteristics of inflammatory cytokines and the correlation with the clinical data of liver function were analyzed,and the related statistical analysis was carried out at the same time.Results:1.In the serum samples of 53 AILD patients,20 cytokines related to AILD process were detected and analyzed.It was found that there were significant differences in the expression of IL-2,IL-4,IL-10,IL-28 A,TGF-β1,TNFβ in the serum of AIH-PBC OS and PBC subjects,healthy controls and hepatitis b controls.2.The results of high-throughput antibody microarray showed that AIH-PBC IL-4 and TGF in serum of patients with overlap syndrome in beta 1 expression level was significantly higher than that of group PBC(p<0.05)AIH-PBC,the amount of overlap level was significantly higher than that of healthy control group,the expression of IL-4 in serum of patients with the syndrome(p<0.05),IL-4 and TGF in serum of patients with PBC beta 1 expression level was significantly lower than that of the healthy control group(p<0.05).3.In the test of 39 AIH-PBC OS patients whose serum autoantibody sp100 and gp210 expression,the difference in the expression of inflammatory factors,and it is found that there is a significant difference in the expression of IL-4,IL-22,TGF-β1 in different autoantibody sp100 AIH-PBC OS patients,and TGF-β1 in different autoantibody gp210 AIH-PBC OS patients express quantity exist significant differences between groups.4.The expression of TGF beta 1 in the serum of PBC patients was negatively correlated with the level of ALT and AST.In AIH-PBC OS patients,the expression of IL-4 is positively correlated with the level of AMA-M2(r=0.327,p<0.001),and the expression of TGF-β1 is negatively correlated with the level of AMA-M2(r=-0.157,p=0.027).5.ROC curve analysis showed that the area under the IL-4 curve was 0.721,and the critical point was IL-4 0.28 pg/m L,the area under the TGF-β1 curve was 0.744,and the critical point was TGF-β1 401.08 pg/m L,the area under the AMA-M2 curve was 0.600,and the critical point was AMA-M2 positive,with AIH-PBC OS as a diagonal.With HBV as the diagonal,the area under the IL-4 curve was 0.745,and the critical point was IL-4 7.45 pg/m L,the area under the TGF-β1 curve was 0.690,and the critical point was IL-4 566.44 pg/m L.Conclusion:In the study of the expression of 20 helper T cell associated inflammatory factors in the serum of 53 patients with AILD,the result was that there was not one or one type of cytokines that played a role in the development of AILD.Many cytokines play a role in the process of fibrosis and inflammation of AILD disease and play a role of mutual promotion or antagonism in the development of AILD.AIH-PBC OS patients are in a high immune state,and the regulation of IL-4 inflammatory factor secretion by IL-4/SATA6 signaling pathway plays a certain role in alleviating the progression of AIH disease.For another,the Th17/Treg imbalance may be the causes of liver autoimmune dysfunction in PBC patients.Since the patients with liver function damage degree,the more serious the imbalance of Th17/Treg state.At the same time,the detection of IL-4 and TGF-β1 expression and the detection of autoantibody levels are helpful to the diagnosis of AILD patients in a timely manner,especially for the diagnosis of viral hepatitis patients with positive autoantibody detection.To a certain extent,the further study of different cytokines is helpful to explore the pathogenesis of AILD,but also to guide the clinical diagnosis of AILD patients in time.
