Objective:(1)To explore the clinical effect of high frequency repetitive transcranial magnetic stimulation(rTMS)in children with a diagnosis of attention-deficit hyperactivity disorder(ADHD).(2)To explore the effect of medicine and high frequency rTMS treatment on the serum level of brain derived neurotrophic factor(BDNF)in children with ADHD.Methods: 34 children with ADHD were treated with methylphenidate 18mg/d and randomly divided into real rTMS group(n=17)and sham rTMS group(n=17).The rTMS of 10 Hz on right prefrontal lobe was performed in study group 4 weeks and sham stimulation was used in sham group. The Chinese version of the Swanson Nolan and Pelham,Version IV Scale-Parent form for ADHD(SNAP-IV)and BDNF serum were assayed before and after 4 weeks’ treatment, the data of two groups were collected and analyzed by SPSS17.0.Results:(1)The SNAP-IV whole scale, subscales and the serum level of BDNF before the treatment between real rTMS group and sham rTMS group had no significantly differences(P>0.05).(2)After 4 weeks’ treatment, the SNAP-IV whole scale and subscales of real rTMS group and sham rTMS group were much lower than that of before(P<0.05),while the serum level of BDNF was much higher than that of before(P<0.05).(3)Compared to the sham rTMS group, the reduction of SNAP-IV total score,attention-deficit score,oppositional-defiant score and the increase of serum BDNF level in real rTMS group had no significantly differences(P>0.05),but the hyperactivity-impulsion score had an obvious decline(P<0.05).(4)The reduction of SNAP-IV total score,hyperactivity-impulsion score and oppositional-defiant score before and after treatment in all ADHD children was positively correlated with the increase of serum BNDF level(P<0.05).Conclusion:(1)Compared to the methylphenidate, the treatment effect of methylphenidate combined with high frequency rTMS on ADHD children had no remarkable differences.(2)After treatment, the symptoms of children with ADHD had a significantly decline, while the serum level of BDNF had an obvious increase,so serum BDNF level may be able to predict the treatment efficacy of ADHD,but High frequency rTMS can’t obviously increase the serum level of BDNF in children with ADHD.
