Objective: To evaluate the value of regular ultrasound(US)combined with contrastenhanced ultrasound(CEUS)in diagnosing thyroid nodules coexisting with Hashimoto’ s thyroiditis(HT).In order to improve the diagnostic accuracy and provide imaging basis for the choice of clinical treatment.Methods: Retrospective analysis in January 2013-December 2016 in the first affiliated hospital of xinjiang medical university,175 patients with HT were diagnosed by clinical or pathological,and combination of benign and malignant thyroid nodules,including 32 cases of male,female 143 cases;24 ~ 69 years old,average(45.74-9.25)years old.161 thyroid nodules coexisting with HT confirmed by surgical pathology were retrospectively analyzed,Calculated and compared the sensitivity(Sen),specificity(Spe),positive predictive value(PV+),negative predictive value(PV-),positive likelihood ratio(LR+),negative likelihood ratio(LR-)and their 95%CI of US+CEUS and US’s in diagnosing thyroid nodules coexisting with HT at the same time.Results: 1)The pathological classification: 161 thyroid nodules in this study,119 malignant nodules,including 118 papillary carcinoma,1 medullary carcinoma,42 Benign nodules,including 4 nodular goiter,38 HT nodules;2)US and US + CEUS in the diagnosis of benign and malignant thyroid nodules under the background of HT: characteristics of benign and malignant nodules at the border,aspect ratio,internal echo,minute calcification and compare the differences between blood flow distribution have statistical significance(P<0.05).Benign and malignant nodules in ultrasound imaging enhancement pattern,enhance the level and fade level between comparative differences are statistically significant(P<0.05);3)The Sen,Spe,PV+,PV-,LR+,LR-and their 95%CI of US+CEUS in diagnosing thyroid nodules coexisting with HT was 95.8%(90.0%~98.4%),88.1%(73.6%~95.5%),95.8%(92.3%~99.3%),88.1%(78.3%~97.9%),8.05(3.53~18.3),0.05(0.02~0.11);The Sen,Spe,PV+,PV-,LR+,LR-and their 95%CI of US was 88.2%(80.7%~93.2%),81.0%(65.4%~90.9%),92.9%(88.2%~97.6%),70.9%(58.0%~83.8%),4.6(2.5~8.7),0.15(0.09~0.24);The various indicators of US+CEUS were higher than US’s,Differences were calculated(P<0.05).Conclusion: US+CEUS can improve the diagnosis accuracy of thyroid nodules coexisting with HT than merely US. |
