| Purpose:To analyze thedifferences of clinical features and prognosis betweenfrequent exacerbations phenotype and infrequent exacerbations phenotype..ln order to improve the understanding of the two and help in differential diagnosis and adjuvant therapy.Methods:This was a retrospective case series study.forty-three patients with frequent exacerbations and fifty-one patients with infrequent exacerbations were enrolled in the study.Collect clinical data of frequent exacerbationsand infrequent exacerbations,including general conditions(gender,age,smoking history,etc.),laboratory tests(blood routine,ESR,C-reactive protein,microbiological examination,etc.),imaging data,efficacy indicators(case fatality rate,hospitalized days,etc.)).Perform statistical analysis and comparison the differences between the two groups by using the SPSS20.0.Results:In clinical symptoms,the rate of moist rale in frequent exacerbations was higher in patients in infrequent exacerbations,and had statistically significant differences(P<0.05).In laboratory tests,the proportion of increased leukocytećpercentage of neutrophils,and increase in C-reactive protein(CRP)was higher in frequent exacerbationswas than infrequent exacerbations.On the microbiological examination.there was a higher proportion of Fungal and Non fermented Gram-negative bacteria in frequent exacerbations than infrequent exacerbations(P<0.05).In terms of efficacy,the initial treatment failure rate was higher in frequent exacerbations than infrequent exacerbations(P<0.05).Conclusions:Patients with frequent exacerbations phenotype chronic obstructive pulmonary disease,The inflammatory response was heavier,the condition was more critical,and was more susceptible to fungal and non-fermenting gram-negative bacterial.The initial treatment failure rate is higher and the prognosis is worse. |
