| Objective Analysis of the safety,feasibility and therapeutic effect of allogeneic bone grafts combined with internal fixation in the treatment of intervertebral space infection after thorough removal of lesions compared with autologous bone graftin g alone.Methods A retrospective analysis of 92 cases of thoracolumbar vertebral tu berculosis vertebral surgery in our hospital from January 2012 to December 2015 was performed.The data were statistically analyzed and the patients were collected before surgery,6,12 months patients with positive lateral X-ray of the spine,the t wo groups of patients after surgery,bone fusion,Cobb angle changes,the clinical efficacy evaluation and analysis.Results Ninety-two patients with lumbar and verteb ral tuberculosis infection underwent surgical treatment.All patients were divided int o group A(A group)and group B(B group).A group of 42 patients,aged 2-72 y ears,mean 43 years old,intraoperative implantation of allograft bone and internal f ixation.Group B,50 cases,aged 15-70 years,mean 39 years old,intraoperative im plantation of excised spinous process or lamina and use bite forceps bite into pelle ts,followed up for 18 to 36 months,an average of 22 months.Imaging showed n o internal fixation postoperative loosening,rupture,displacement;no recurrence of i nfection,Cobb angle in both groups have been improved to some extent,preoperat ive Cobb angle in the 5°-45°,the average preoperative Cobb angle was 31.23° ± 5.38° and the Cobb angle was 15.71°±2.34°.The Cobb angle at the final follow-up was 17.23 °±2.47 °.The mean preoperative Cobb angle was 11°-55° in Group B with an average of 30.7°±3.47°.14.65° ±2.12°.The Cobb angle at the last followup was 16.33°±2.41°.There was no obvious loss of correction angle.The Cobb follow-up was 16.33°±2.41°.There was no obvious loss of correction angle.The Cob b angle of the two groups was significantly improved compared with that before o peration.There was no failure of settlement,collapse and fusion of bone graft.The rate of fusion was 21.4%(9/42)at 3 months after operation and 66.7%(28/42)a t 6 months after operation.The fusion rate at 12 months after operation was 95.2%(40/42)).The fusion rate was 30%(15/50)at 3 months after operation and 84%(42/50)at 6 months after operation.The fusion rate was 100% at 12 months a fter operation.The operation time,intraoperative blood loss,hospitalization days,J OA score,C-reactive protein,erythrocyte sedimentation rate and ASIA grading were compared between the two groups.Conclusion The fusion effect of allogeneic bon e grafts is basically the same as that of autologous bone grafts,but allogeneic bon e grafts can not only make up for the insufficient amount of autogenous bone graf ts in intervertebral space infection.The fusion effect was basically the same as that of using autologous bone granules alone.The clinical effect of allografts in the tr eatment of thoracolumbar tuberculosis intervertebral space infection could be confir med,and the average number of days in hospital was less and the safety of alloge neic bone grafts was lower.Reliability can be recognized. |
