A Clinical Study Of Benign Or Malignant Ultrasound BI-RADS 4a Small Breast Nodules

Objesctive:To evaluate the feasibility of follow up therapy to partial BI-RADS 4a patients through analyzing the evidences to differentiate the benign from malignant small breast nodules.Methods:We retrospectively analyzed the Ultrasound image features and related clinical data of 105 patients who were diagnosed with BI-RADS 4a small breast nodules by ultrasound and underwent surgery in our hospital from September 2015 to December 2017.Patient’s age,age at menarche,history of benign breast disease,location of nodule,body mass index,multiple nodules,the diameter of nodule,nodule margin,nodule sharp,ascularity in nodule and magnetic resonance imaging was analyse to evaluate the relationship between these evidences to small breast nodules.?~2 test or Fishers exact test was used for univariate analysis,then stepwise logistic regression was used for multivariate analysis.Last,the study factors of fibroadenoma or adenosis was compared to malignant small breast nodule’s by?~2 test or Fishers exact test.Results:Univariate analysis and multivariate analysis showed that no circumscribed margin and blood flow within the nodule in nodule’s ultrasonography were the evidence of malignant BI-RADS 4a breast nodules(P<0.05).In fibroadenoma group,the proportion of the patients with circumscribed margin in nodule’s ultrasonography,no blood flow within the nodules and the magnetic resonance imaging indicate that the nodule is benign was significantly higher than proportion in malignant group(all P<0.05).In adenosis group,the proportion of the patients with multiple nodules,nodule’s diameter<1cm and the magnetic resonance imaging indicate that the nodule is benign was significantly higher than proportion in malignant group(all P<0.05).Conclusion:Circumscribed margin in nodule’s ultrasonography,no blood flow within the nodules and the magnetic resonance imaging indicate that the nodule is benign were the evidence of benign BI-RADS 4a small breast nodules,mainly fibroadenoma.Multiple nodules,nodule’s diameter<1cm and the magnetic resonance imaging indicate that the nodule is benign were the evidence of adenosis.Patients of these two type nodules can choose close follow up therapy,instead of immediate biopsy or surgery.
