Analysis Of Thespinopelvic Sagittal Parameters Of Spondylolysis And Isthmic Spondylolisthesis

By measuring the lumbar spondylolysis patients,patients with isthmic spondylolisthesis of the spinopelvic sagittal parameters,respectively,and the statistical analysis of the parameter value of healthy crowd found three kinds of population differences and correlation of various parameters,To guide the selection of surgical methods for patients with lumbar spondylolysis.Methods:Collection of the xinjiang military region general hospital spinal surgery in November 2010 to December 2017 were40 patients with simple lumbar spondylolysis,40 patients with spondylolysis sex lumbar spondylolysis imaging data(full spine X-ray film)and to measure the spine and pelvic sagittal balance parameters.Including: lumbar lordosis Angle(LL),after tthoracic kyphosis Angle(TK),pelvic incidence Angle(PI),pelvic tilt Angle(PT),sacral slope Angle(SS),spinal Sagittal vertical axis(SVA),etc.Results:(1)lumbar spondylolysis except in the normal group after thoracic lobe(TK)decreases,and lumbar lordosis Angle(LL),pelvic incidence Angle(PI),pelvic tilt Angle(PT),sacral slope Angle(SS)are increasing in the normal group,the PI,the SS increase statistically significant(P<0.05);(2)isthmic spondylolisthesis group compared with normal group after thoracic kyphosis(TK)decreased and was statistically significant(P<0.05),lumbar lordosis Angle(LL)pelvic incidence Angle(PI),pelvic tilt Angle(PT),sacral slope Angle(SS)are increasing and it has statistical significance(P<0.05);(3)isthmic spondylolisthesis pelvic incidence Angle detachment group PI,sacral slope Angle SS were significantly increased in the lumbar spondylolysis group difference was statistically significant(P<0.05)lumbar spondylolysis set of spine and pelvis sagittal parameters Pearson correlation analysis,results show that the PI with the SS show positive correlation(r=0.361,P<0.05),with lumbar spondylolysis,had the spinopelvis sagittal balance parameters of Pearson correlation analysis results,results show that the PI with the SS show positive correlation(r =0.436,P<0.05),PI and PT show positive correlation(r=0.411,P<0.05),PI and LL show positive correlation(r=0.429,P<0.05),SS and LL show positive correlation(r=0.407,P<0.05).Conclusion:In this study,we found that the sagittal parameters of the spinopelvic of lumbar spondylolysis and isthmic spondylolisthesis were different from those of normal parameters.There were statistically significant differences between the isthmic spondylolisthesis group PI and SS in the lumbar spondylolysis and the normal group(P<0.05).There was positive correlation between PI and SS of lumbar spondylolysis and isthmic spondylolisthesis,When SS 45 °or greater in patients with lumbar spondylolysis vertebral bodies increases significantly in the shear force,surgery tend to adopt intervertebral bone graft fusion,which can effectively prevent increase of the shear stress lead to corresponding changes of intervertebral disc seriously,so as to reduce the risk of surgical treatment again.
