Objective: To investigate the plasma homocysteine(Homocysteine,Hcy)the correlation between the level and degree of dementia and dementia,and explore the levels of plasma Hcy and Alzheimer's disease(Alzheimer's,disease,AD),vascular dementia(Vascular dementia,VD)and Parkinson disease(Parkinson diseasewith,dementia,PDD)the relationship between.Materials and methods: Case group: A total of 106 patients with dementia in the elderly neurology department of the First Affiliated Hospital of Nanjing Medical University from December 2014 to December 2017 were selected as 106 cases,46 males and 60 females.The sex,age distribution and education level of the hospitalized patients were statistically analyzed,and blood plasma Hcy was measured;and MMSE(Mini-mental state).Examination,MMSE)scale,according to the score,the senile dementia patients were divided into mild,moderate and severe dementia group,with 45 cases of mild dementia,40 in moderate dementia group and 21 in severe dementia group.All dementia patients were divided into AD,VD and PDD groups based on the fourth edition of the American psychiatric disorders diagnosis and statistics manual and the PDD diagnostic guide(MDS)established by the international society for dyskinesia(MDS).Among them,there were 36 cases in group AD,33 in VD group,17 in PDD group and 30 in other dementia types.All participants were informed consent;all patients were excluded from severe heart,liver and kidney dysfunction,tumors,autoimmune diseases,taking various vitamins and so on.The normal control group consisted of 90 patients,35 males and 55 females,with an average age of 73.57 + 7.14 years,all of whom were from the same clinic.There was no significant difference in age,sex composition and education level between the two groups.Methods: all the subjects were collected venous blood in the early morning of the next day,and the concentration of Hcy was detected by fluorescence chemiluminescence.The MMSE scale was completed within 24 hours in all cases.Statistical analysis: t test was used to analyze whether there was a difference in the level of Hcy between the case group and the control group;through the linear relationship,the relationship between the Hcy levels of the patients with different dementia level and the correlation with the MMSE was investigated.The effects of Hcy on the types of dementia were compared through the single factor analysis of variance.Results: Compared with the control group,there was no significant difference in age,sex and cultural level(P > 0.05).The level of plasma Hcy in the case group was 17.48±6.18umol/L,and the level of plasma Hcy in the control group was 13.69 ±5.08 mu mol/L.The level of plasma Hcy in the case group was significantly higher than that of the control group(t=5.04,P < 0.05,Table 2),and the plasma HHcy in the case group.There were 67 cases(63.2%)and 31 cases(34.4%)in the control group.The difference of plasma HHcy rate in the two groups was statistically significant(X2 = 15.32,P<0.05,Table 2).The results showed that the plasma HHcy was associated with the onset of Alzheimer's disease.The degree of dementia in senile dementia was compared: the plasma Hcy concentration in the mild group was 15.32±5.08?mol/L,the concentration of plasma Hcy in the moderate group was 18.58±5.54 mol/L,and the concentration of Hcy in the severe group was 20.00±8.04 mol/L.The results showed that the difference of plasma Hcy level between the patients with different dementia degree had the significance of the total plasma Hcy level(P<0.05,table 3).Correlation analysis between plasma Hcy level and MMSE score showed that there was a significant negative correlation between plasma Hcy level and MMSE score(r=-0.45,P<0.05,Figure 1).The different types of senile dementia: the plasma Hcy level in AD group was 17.95±5.08umol/L,the plasma Hcy level was 20.99±5.92 u mol/L in group VD,and the level of Hcy in PDD group was15.66±4.31 u mol/L.The results showed that the Hcy level in the VD group was significantly higher than that of AD and other groups(Table 4).There was no significant difference in plasma Hcy level between the two groups(P > 0.05,Table 4).Conclusion: 1.HHcy is associated with the onset of Senile dementia.2.The level of Hcy in elderly patients is related to dementia.The higher plasma Hcy level,the lower the cognitive level score,that is,Hcy level is negatively correlated with MMSE score.3.the plasma Hcy level of VD patients was significantly higher than that of AD and PDD groups..4.For elderly patients with HHcy,cognitive function can be assessed routinely and dementia can be screened for early detection and early treatment.
