Objective:The purpose of this study was to evaluate the difference in the clinical efficacy of tinnitus between acupuncture therapy of regulating qi circulation of qiao-organ and acupuncture therapy with conventional methods.Then we preliminarily discussed the adaptive clinical syndromes of acupuncture therapy of regulating qi circulation of qiao-organ.Methods:There were 66 cases of tinnitus that met the inclusion criteria.They were randomly divided into 33 cases of the regulating qi circulation of qiao-organ acupuncture group and 33 cases in the conventional acupuncture group.Finally,32 cases of the regulating qi circulation of qiao-organ acupuncture group and 30 cases of the conventional acupuncture group finished the research.Both groups selected auricular acupuncture points(Ermen SJ 20;Tinggong ST 19;Tinghui GB 2;Yifeng SJ 12)and combined with acupuncture points that was selected through TCM syndrome differentiation theory.Based on the treatment of the conventional acupuncture group,the regulating qi circulation of qiao-organ acupuncture group was added to the acupoint group of regulating qi circulation of qiao-organ(Baihui DU 20;Danzhong RN 17,Qihai RN 6;Zusanli ST 36;Waiguan SJ 5).Danzhong acupoint needle is 45 degree Angle up.Waiguan acupoint down the needle is 15 degree Angle.Both two groups were needling conventionally without acupuncture manipulation.Needle retaining time was 30 minutes.Patients in both groups received treatment for 6 weeks,5 times a week.2 weeks were a course of treatment.A total of 3 courses of treatment were included.In the end,the difference in efficacy of two treatments in tinnitus was evaluated by comparing the severity score and the grade of tinnitus before and after treatment.Results:(1)After treatment,we compared the clinical efficacy of two groups.Of 32 patients in the regulating qi circulation of qiao-organ acupuncture group,7 were cured,10 had experienced improvement,9 were effective and 6 were ineffective.The total effective rate was 81.25%.Of 30 patients in the conventional acupuncture group,2 was cured,6 had experienced improvement,13 were effective and 9 were ineffective.The total effective rate was 70.00%.On wilcoxon’s test,z =-2.047,P=0.041,which indicated that the two groups had a significant difference in the clinical efficacy.(2)Comparison of tinnitus severity scores between two groups before and after treatment:There was no significant difference in tinnitus severity scores between the two groups of tinnitus patients before treatment(P>0.05).The tinnitus severity scores of two groups of tinnitus patients were significantly different from those before treatment(P<0.05).After treatment,the degree of tinnitus severity score in the regulating qi circulation of qiao-organ acupuncture group was significantly lower than that in conventional acupuncture group,and the difference was statistically significant(P<0.05).(3)Comparison of tinnitus classification of two groups of tinnitus patients before and after treatment:Before treatment,there was no significant difference in tinnitus grading between the two groups(P>0.05).The tinnitus classification of the two groups of tinnitus patients before and after treatment was compared,there was a significant difference between the regulating qi circulation of qiao-organ acupuncture group(P < 0.05),and there was no significant difference between the conventional acupuncture group(P > 0.05).The tinnitus classification of the regulating qi circulation of qiao-organ acupuncture group and the conventional acupuncture group after treatment,there was a significant difference between the two groups(P<0.05).(4)Comparison of clinical efficacy of different syndromes in the regulating qi circulation of qiao-organ acupuncture group:4 patients with hepatobiliary fire type tinnitus: 3 cases were cured and 1 case was markedly effective.10 patients with phlegm within fire type tinnitus:2 cases were cured,4 cases were markedly effective,3 cases were effective and 1 case was invalid.12 patients with spleen deficiency type tinnitus:2 cases were cured,5 cases were markedly effective,3 cases were effective and 2 cases were invalid.6 patients with kidney essence deficiency type tinnitus: 0 case were recovered,0 case was markedly effective,3 cases were effective and 3 cases were invalid.Conclusion :(1)This study indicates that both acupuncture treatments have good therapeutic effect on tinnitus.Moreover,the effective rate of acupuncture therapy with regulating qi circulation of qiao-organ is higher than that of conventional acupuncture.(2)Both treatments of acupuncture can reduce and improve the total score of tinnitus and tinnitus classification.However,the acupuncture with regulating qi circulation of qiao-organ is superior to conventional acupuncture.(3)The acupuncture treatment of regulating qi circulation of qiao-organ needs to be tested in the clinical research of large samples if we want to explore the principal indication of this treatment.
