ObjectivesTo investigate the direct economic burden and the quality of life of the elderly patients with chronic respiratory diseases in the city,and analyze the unfavorable factors affecting the direct economic burden and quality of life of the elderly patients.A deeper understanding of the social and psychological needs of the elderly aims to reduce the burden of disease on elderly patients with chronic diseases,and provide a reference and basis for solving a series of problems arising from the aging process in China.MethodsFrom October 2016 to June 2017,a questionnaire survey was conducted on 384 elderly inpatients in the First Affiliated Hospital of Jinzhou Medical University.Using the self-designed General Information Questionnaire for Chronic Patients,the Morisky Medication Compliance Questionnaire(MMAS-8),The Chinese version of the EuroQol-5 Dimensions(EQ-5D),and the Chronic Disease Self Efficacy Scale(SECD6),The Geriatric Depression Scale(GDS-15)was used to survey elderly patients with chronic respiratory diseases.Data was analyzed by SPSS19.0 software.Descriptive analysis of frequency,mean,standard deviation,and median for general survey data;analysis of scores of elderly chronic respiratory disease patients with different demographic characteristics;using Kruskal-Wallis-H non-parametric test method,and using the SNK method to perform a pairwise comparison;the statistical analysis of the factors related to direct economic burden,self-administration behavior,self-efficacy,and emotional depression in elderly patients with chronic respiratory diseases was analyzed using Spearman rank correlation;multiple stepwise regression was used.Analytical methods analyze the direct economic burden and the quality of life of elderly patients with chronic respiratory diseases.With P < 0.05 was statistically significant.ResultsA total of 425 questionnaire was distributed and effective questionnaire 384,The effective recovery rate was 90.35%.60-70-year-old elderly patients accounted for 51.30%,the average length of stay in hospital(10.02±4.25)days;Hospitalization cost was(8816.5±4207.1),which the pharmaceuticals cost is the major cause of direct economic burden,accounting for 49.22% of the total cost.The factors influencing the direct economic burden of elderly patients with chronic respiratory diseases include monthly income,days of hospitalization,number of chronic diseases combined,health score,medication compliance level,depression level,self-efficacy score,scope of major medical insurance reimbursement,and efficacy of disease treatment(P < 0.05),in which the hospitalization days had the greatest impact on the direct economic burden of elderly patients with chronic respiratory diseases.The influencing factors of quality of life of elderly patients with chronic respiratory diseases include self-care ability,hospitalization days,self-efficacy,age,medication compliance(P < 0.05).affecting the quality of life in elderly patients including hospitalization days,aged,self-efficacy,self-care ability,medication compliance levels(P < 0.05).ConclusionsThe medical expenses of elderly patients with chronic respiratory diseases are relatively high.The major medical insurance reimbursement model for chronic respiratory diseases in the region is not yet perfect,which is not conducive to reducing the direct economic burden of elderly patients with chronic disease.Elderly patients with severe direct economic burdens have a low level of self-efficacy and a high incidence of depression,which has an adverse effect on the quality of life of the elderly.In the survey of elderly patients with respiratory diseases in the region,it was found that elderly patients with high levels of direct economic burden have low compliance with medications and also have lower levels of quality of life.The importance of chronic respiratory diseases should be increased,and the popularization of knowledge about medication for elderly patients with chronic diseases should be strengthened,the self-administered drug administration level of the elderly should be improved,psychological support should be strengthened,and the quality of life of elderly patients with chronic diseases should be continuously improved.
