Akylosing Spondylitis(AS)is a chronic progressive inflammatory disease that mainly affects the joints of the spine and can affect the sacroiliac joints, peripheral joints, and tendon terminals to varying degrees and can also be associated with extra-articular manifestations disease. Objective: To observe the clinical efficacy and safety of Chinese herbal fumigation combined with Western medicine treatment for kidney-deficiency type ankylosing spondylitis through clinical case-control study from the perspectives of main symptoms, signs, laboratory indicators, adverse reactions, and clinical efficacy. The combination of Western medicine and Chinese herbal fumigation provides a reliable clinical basis to treat AS,which is conducive to the popularization and application of fumigation therapy of Chinese medicine.Method: Sixty AS patients admitted to the Rheumatology Department of Chinese People’s Liberation Army of Wuhan General Hospital were randomly divided into fumigation group and control group, with 30 cases in each group. The control group was treated with Celebrex plus sulfasalazine, and the fumigation group was treated with fumigation and steaming of our own homemade fumigation prescription on the basis of the control group.Observe and record the main symptom indexes(VAS pain score,BASDAI index,BASFI index),major physical indicators(pillow wall distance, thorax extension,Schober test)and laboratory indexes(ESR,CRP)before and after treatment for 3 months, using SPSS19.0 statistical software to analyze the changes before and after treatment of various indicators of change, record the number of cases of various adverse reactions during treatment, the efficancy of TCM syndrome and western medicine, and then use statistical software to analyze and compare these diferences.Result:1.Comparison of main symptom indexes: VAS pain score,BASFI,and BASDAI in the fumigation group before treatment were not significantly different from those in the control group(P>0.05).Before and after treatment, the difference of the above three indicators within the two groups was statistically significant(P<0.01).After treatment, there was a statistically significant difference between the fumigation group and the control group(P<0.05).2.Comparison of main physical indicators: There was no significant difference between the fumigation group and the control group in the occipital distance, thorax extension, and Schober test(P>0.05).Before and after treatment, the difference of the above three indicators within the two groups was statistically significant(P<0.01).After treatment, there was a statistically significant difference between the fumigation group and the control group(P<0.05)..3.Comparison of laboratory indicators: There was no significant difference in ESR and CRP between the two groups before treatment(P>0.05).Before and after treatment, there was significant difference between the two groups in ESR and CRP groups(P<0.01).After treatment, the difference between ESR and CRP groups was statistically significant(P<0.05).4.Comparison of safety indicators: There were no significant differences in the adverse reactions between the two groups during treatment(P>0.05).5.Comparison of TCM syndromes: After treatment, the efficacy of TCM syndromes in the two groups was compared, and the difference was statistically significant(P<0.05).6.Comparison of the efficacy of Western medicine ASAS20: Compared with the efficacy of ASAS20 after treatment, the difference was statistically significant(P<0.05).Conclusion:1.Traditional Chinese medicine fumigation combined with Western medicine treatment of kidney deficiency and cold type AS can significantly improve the patient’s main symptoms, signs and laboratory indicators, and the improvement degree and efficacy are better than simple western medicine therapy.2.There was no significant increase in adverse reactions between Chinese medicine fumigation combined with western medicine treatment of AS and western medicine alone. |
