Objective:The myocardial activity was assessed quantitatively by measuring the myocardial concentration and standard iodine concentration in the anterior and anterior-septal wall of the left anterior descending branch of the coronary artery by measuring the Dual-energy computed tomography scan, that is to say,"One-Stop Shop"including coronary computed tomography angiogtaphy and myocardial energy analysis,to the suspected and diagnosed patients with coronary heart disease.By using myocardial energy analysis to identify the difference of normal, ischemic and infarcted myocardium.Combining coronary computed tomography angiography with myocardial energy analysis to assess the association between coronary stenosis and myocardial ischemic states. Methods:Clinical data were achieved from patients who were admitted to the department of cardiovascular in Chen zhou First People’s Hospital,South China University during from March 2016 to January2017 in University. The diagnosis were made by two experienced cardiovascular physicians according to the patient’s clinical symptoms,ECG,radionuclide myocardial examination, cardiac troponin, ultrasonic cardiogram. The patients were divided into three groups:normal myocardium, myocardial ischemia, myocardial infarction. The myocardium showed normal in 35 cases(group A),ischemic in 15 cases(group B1)and infarcted in 9 cases(group B2).All patients were monitored by rigorous breathing training, using prospective ECG gating technology and Dual-energy computed tomography checking technique to create a data packet named"DATA"and data was analysed on the AW4.6 working platform. The iodine concentration and the iodine concentration in the aorta of the same level were calculated in the anterior wall and the anterior-septal wall of the left anterior descending branch. The data of iodine concentration, standard iodine concentration and left anterior descending stenosis were measured by SPSS 19 software for data analysis. Results:The concentration of iodine in normal myocardium(group A)was(25.67±3.80)100ug/cm~3;the concentration of iodine in ischemic myocardium(group B1)was(23.90±2.97)100ug/cm~3;the iodine concentration of infarcted myocardium(group B2)was(10.60±4.96)100μg/cm~3.There was a significant difference in the concentration of iodine between normal and infarcted myocardium, and the ischemic myocardium between infarcted myocardium(P<0.05).There was no significant differences in iodine concentration between normal myocardium and ischemic myocardium(P>0.05).The myocardial concentration of infarcted myocardium was significantly lower than that of normal myocardium and ischemic myocardium.Standard iodine concentration refers to the ratio of myocardial iodine concentration to aortic iodine concentration in the same level of the left anterior wall and anterior-septal wall of myocardial iodine concentration. The standard iodine concentration in the left anterior wall and anterior-septal wall of myocardial iodine concentration. The standard iodine concentration in the normal myocardium(group A)was 0.16(0.15,0.18).The standard iodine concentration in ischemic myocardium(group B1)was 0.12(0.11,0.13).The standard iodine concentration in the infarcted myocardium(group B2)was 0.10(0.11,0.13).There was statistically significant difference in the standard iodine concentration between normal myocardium and ischemic myocardium, and so as between normal myocardium and infarcted myocardium(P<0.05).There was no significant differences in the standard iodine concentrationbetweenischemicmyocardiumandinfracted myocardium(P>0.05).The levels of standard iodine in ischemic myocardium and infarcted myocardium were significantly lower than those in normal myocardium. The data revealed that the correlation between the standard iodine concentration, the iodine concentration and the degree of stenosis of the coronary artery was not statistically significant by Spearman rank correlation analysis(P>0.05).Conclusion:Using Dual-energy computed tomography scan to identify the myocardial concentration of iodine, standard iodine concentration in the left anterior wall and anterior-septal wall determination and analysis of the study can distinguish between normal myocardium, ischemic myocardium and infarct myocardium to assess myocardial activity. The iodine concentration, the standard iodine concentration and the degree of left anterior descending coronary artery stenosis measuring by Dual-energy computed tomography scan were evaluated, suggesting that there is no significant correlation between the degree of myocardial ischemia and the degree of coronary artery stenosis.
