Prevalence And Risk Factors Of Senile Dementia In Elderly People Over 65 Years Old In Yueyang Tower District Of Yueyang City

Objective Through the cross sectional study on senile dementia of elderly that 65 years old and above in in Yueyang Tower District of Yueyang City,understood the prevalence of senile dementia,explored the risk factors,to provide reasonable suggestions for the prevention and treatment of senile dementia in the community.Method Extract 5 communities in 15 communities of in Yueyang Tower District of Yueyang City using random cluster sampling method.Conduct questionnaire survey for the elderly adopt random sampling method in the above 5 communities.Conduct neuropsychological assessment for the elderly using Mini Mental State Examination(MMSE),MoCA scale of Changsha version,Center for Epidemiological Survey Depression Scale,Hachinski Inchemic Score and self made general table.Diagnosis dementia using DSM-IV;Alzheimer's disease and vascular dementia were diagnosed with NINCDS/ADRDA.Data analysis was performed using SPSS 18.0,count data use rate or composition ratio to express,test comparison with?~2;The risk factors of senile dementia were analyzed by Univariate and Multinomial Logistic Regression analysis.Result(1)140 cases were diagnosed of senile dementia,the prevalence was 5.38%;The ratio of AD,VD and mixed dementia were respectively 75.71%,17.14%,and 7.15%.(2)The prevalence of female dementia was 6.39%,and it was higher than that of male dementia which was 4.25%(P<0.05).(3)The prevalence of dementia increased with aging.Age group is divided 5 years as one age group,the prevalence rate of 65 to 69 years old age was 1.22%,while it was 16.7%in the elderly people over 85 years(P<0.05).(4)The prevalence of senile dementia was significantly correlated with the level of education.The education level is divided into 5 stages(illiteracy,primary school,junior high school,senior high school,college or above),the prevalence rate of senile dementia in the illiteracy elderly was 8.28%,while it was 0.89%in the elderly people in college or higher education(P<0.05).(5)The univariate analysis of risk factors for senile dementia showed 11 factors associated with senile dementia(P<0.05),respectively are:gender,age,education,stroke history,hypertension,diabetes,hyperlipidemia,head trauma history,depression,smoking,hobbies.(6)Multi factor Logistic regression analysis showed,female,age,stroke history,head trauma history,hyperlipidemia,smoking increased the risk of senile dementia,while highly-educated,widely hobbies are protective factors.Conclusion The prevalence rate of senile dementia in the elderly over 65 years old in Yueyang Tower District of Yueyang City was 5.38%.With the age increased,the elderly have the more risk.The aging of population in China is very serious,it should cause social attention;Female,age,stroke history,hypertension,diabetes,hyperlipidemia,head trauma history,depression,smoking may be risk factors for senile dementia,and highly-educated,widely hobbies are protective factors.Attention should be paid to the intervention and prevention of senile dementia.
