| Objective:By using the PDCA cycle management model,we evaluated the self-management ability,health management knowledge and self-efficacy of 116 cancer patients with PICC and established the "Interaction Manual of Health Education" to reduce catheter-related complications in PICC patients and extend PICC catheter usage time.Methods:Using self-designed questionnaire,PICC tumor patients were investigated.The research object was 116 cancer patients who underwent PICC post-catheter chemotherapy from May 2015 to November 2015 in Jinan City,Shandong Province.The survey included general information on patients with PICC cancers,self-management skills,self-management efficacy,knowledge of health management and self-monitoring.116 cancer patients with PICC were randomly divided into intervention group and control group by random number table.Control group using conventional health education methods,prior to catheterization and patient communication,signed informed consent,fully evaluate the patient to be placed on the day of catheterization by the researcher told patients and their families PICC maintenance time and retention time,daily life management Of precautions,the purpose and methods of functional exercise,and other PICC-related health management knowledge.At discharge,the investigators again instructed the relevant contents of PICC’s health education management knowledge.Patients are encouraged to consult and respond to catheter-related questions while carrying PICCs.On the basis of routine health education,the intervention group intervened according to the health education plan of PICC catheterized patients constructed on the basis of PDCA cycle management.On the day of catheterization,the day after catheterization,the first week after catheterization,One week after the first three weeks of catheterization health education,each time for 30~40min.During the process of health education for the intervention group,if there are any problems,immediately make the improvement plan,organize the implementation plan,and check whether the expected effect has been achieved;if the improvement fails,adjust the plan;if the improvement succeeds,the action will be institutionalized and flowed through The PDCA cycle is characterized by continuity.It constantly examines existing performance and develops plans to begin new improvements and improve health education.Results:Five weeks after catheterization,two independent samples t-test were used to analyze the health management knowledge scores of two groups of patients.The results showed that the score of the intervention group was higher than that of the control group,with significant statistical difference(P<0.01).Two independent sample t-tests were used to analyze the self-efficacy scores after intervention in both groups.The results showed that the score of the intervention group was higher than that of the control group,with statistical significance(P<0.05).We further used paired t-test to analyze the scores of self-efficacy before and after intervention in intervention group.The results showed that the scores of self-efficacy after intervention were higher than those before intervention(P<0.05).Chi-square test was used to analyze the incidence of catheter-related complications after intervention in both groups.The results showed that the incidence of complications such as phlebitis and catheter-related infections in the intervention group was significantly lower than that in the intervention group(P<0.05).Conclusion:After the PICC is placed in the tumor patients,implementing PDCA cycle management mode and establishing "Interactive Manual of Health Education" can improve patients’ health management knowledge,improve self-efficacy and self-management ability and reduce the incidence of catheter-related complications.The findings of this study for the retention of PICC patients with health education guidance has clinical significance. |
