The Research Of The Neural Function Recovery After The Surgery Of Lumbar Disc Herniation With Cauda Equina Injury Through Electric Acupuncture Treatment

ObjectiveThrough this study,make clearer the clinical effect in the neural function recovery after the surgery of lumbar disc herniation with cauda equina injury through electric acupuncture treatment,expanding the scope of the electric acupuncture clinical application,to explore an simplicity,effective,cheap,convenient characteristics of Traditional Chinese Medicine treatment for the neural function recovery after the surgery of lumbar disc herniation with cauda equina injury through electric acupuncture treatment.General data and Methods1 General data: Collected 64 in-patients accorded with the inclusion criteria of lumbar disc herniation with cauda equina injury from April 2015 to November 2016 in Zhengzhou Orthopaedics Hospital Minimally invasive spine surgery.In accordance with the random number table and registration order the patients were randomly divided into test group 33 cases,control group 31 cases.2 cases were lost in the test group,1 cases were lost in the control group.Among them,61 patients completed the treatment,there were 20 male cases and 11 female cases in the experimental group,Aged 27~71 years(average 43.71±10.42 years);disease duration ranged 3 hours~20 years(average 30.52 months);Also,There were 19 male cases and 11 female cases in the control group,Aged 30~64 years(average 45.73±10.13 years);disease duration ranged 3 hours~20 years(average 40.62 months).According to Tandon types,there were 3 cases of typeⅠ(2 cases because of free type lumbar disc after sprain,1 case because of Huge type lumbar disc after sprain),10 cases of typeⅡ,18 cases of type Ⅲ in experimental group;there were 3 cases of typeⅠ(1 case because of free type lumbar disc after sprain,1 case because of free type lumbar disc after injury,1 case because of Huge type lumbar disc after weight-bearing),11 cases of typeⅡ,16 cases of type Ⅲ in control group.2 Methods:The experimental group and control group were taken fenestration discectomy or laminectomy operation under the same group of doctors according to the patients’ pathogenic condition.The cauda equina nerve were explored,and incised for decompression,and loosened the injured nerve.Under the premise of routine therapy,the control group application methylprednisolone sodium succinate 80 mg/d Within 8 h after the operation,3 days;seven Ye Zao glucoside sodium for injection 10 mg/d,1 w;salvia ligustrazin injection 10 ml/d,monosialotetrahexosyl ganglioside sodium injection 40 mg/d,dosing 2 w.On the basis of the control group treatment given the experimental group electric acupuncture treatment postoperative 1 day.The main acupoint: Zhongliao acupoint,Ciliao acupoint,Huiyang acupoint,Shenshu acupoint,Ganshu acupoint,Shangliao acupoint,Xialiao acupoint,Guanyuan acupoint,Tianshu acupoint,Zhongji acupoint.Matching acupoint: the back Jiaji acupoints,Ashi acupoints and Xunjing acupoints.7 days is one treatment course,1 day off per week,treat 2 courses.To observe the visual analogue scale score,Japanese Orthopaedic Association score,and Shanghai changzheng hospital affiliated to second military medical university fixed Frankle score,and compared them at preoperative and postoperative,1 week after the treatment,2 weeks after the treatment between the experimental group and the control group.comparison and statistical analysis them,to observe the clinical effect in the neural function recovery after the surgery of lumbar disc herniation with cauda equina injury through electric acupuncture treatment.ResultsThe differences of the gender,triggers and Tandon parting between the two groups compared by chi-square test had no statistical significance(P>0.05).Demographic characteristics such as age,height,weight,they ware compared by using independent sample T test,had no obvious difference(P>0.05);The Courses,the VAS scores,Frankle scores and JOA scores before and after the treatment between and within the two groups ware all compared by Rank-sum test,the postoperative VAS scores of the two groups were significantly decreased than the preoperative(P<0.05),But compared the experimental group and the control group had no significant difference(P>0.05);The Frankle scores and the JOA scores before and after the operation,had no obvious change,comparison between the two groups were no significant differences(P>0.05);It was comparable between the two groups.After treatment,the two groups had no cure,In the test group 25 cases ware excellent,4 cases were effective,2 cases ineffective,total effective rate was 93.55%;In the control group 14 cases were excellent,10 cases effective,6 cases ineffective,the total effective rate was 80%.By chi-square test,the electric acupuncture treatments of experimental in the overall clinical efficacy is better than that in control group(P<0.05).Using Rank-sum test compare the VAS score,Frankle score,JOA score between the two groups of postoperative,after 1 week,2 weeks after treatment,postoperative the two groups of VAS score with the passage of time gradually reduce has significant difference(P<0.05).And the experimental group also has obvious advantages in relatively lower than control(P<0.05).After treatment for 1 week,2 weeks the Frankle score of the two groups were gradually increased than the postoperative has significant difference(P<0.05).Also,the treatment group has obvious advantages in Frankle score than the control group(P<0.05).The JOA score of the two groups after treatment compared with postoperative were improved obviously with significant difference(P<0.05).After 1 week,2 weeks treatments the JOA scores of the treatment group has obvious advantages in improving situation than the control group(P<0.05).Conclusion1 Electric acupuncture on the neural function recovery after the surgery of lumbar disc herniation with cauda equina injury has exact curative effect.Electric acupuncture to improve the symptoms of low back pain,promote the lower limb muscle strength,improve the saddle area numbness,promote bladder and bowel function than traditional treatments has significant curative effect.2 lumbar disc herniation with cauda equina injury patients should be diagnosed and operationed early or necessary emergency operation.It will be advantageous to the recovery of the injured cauda equine.Electric acupuncture on Tandon Ⅰ,Tandon Ⅱpatients’ clinical symptoms has obviously effect,but,the effects on the Tandon Ⅲtype and the treatment is not timely,its’ strength is poorer,saddle area is numbness and bladder and bowel function is poor compared with TandonⅠ,TandonⅡhas poorer effectiveness.
