Objectives1、To investigate heparin binding protein‘s diagnostic performance and ensure its effect.2、To compare the diagnostic effects of heparin binding protein and procalciton-in,C-reaction protein and white blood cell count.3、To compare the heparin binding protein’s area under curve in different Cut-off.Methods1、2016 June-July 2016 Tuesday,Thursday,Saturday 9:00-11:00 am,in the People’s Hospital of Jiangxi Provincial People’s Hospital to check the inpatient data,according to the Sepsis 3.0,all included in the following criteria:(1)positive bacterial culture.(2)systolic blood pressure ≤ 100 mmHg;(3)respiratory rate> 22 beats/min.2、The remaining 90 blood samples were excluded according to the following criteria:(1)exclusion of patients with tumor diseases;(2)exclusion of blood samples from patients younger than 18 years;and(3)blood samples from patients with congenital diseases.The remaining 65 blood samples3、In July 2016,Tuesday,Thursday,and Saturday from 9:00 to 11:00 am,blood samples were collected from the physical examination patients in the People’s Hospital of Jiangxi Provincial People’s Hospital.According to Sepsis 3.0,all included in the following criteria:(1)bacterial culture negative;(2)platelets> 150 × 109/L,remaining 59 patients;(3)total bilirubin 1.71-17.1 μmol/L;(4)male creatinine 54-106 μmol /L,female creatinine 44-97 μmol/L.The remaining 39 patients had blood samples.4、The remaining 39 samples in the control group were excluded one by one according to the following criteria:(1)Excluding patients with suspected tumor diseases from the blood samples of 39 patients based on the detection content of tumor markers(such as AFP,CA125,etc.).(2)Blood samples from patients younger than 18 years old.The remaining 37 patients had blood samples.5、The sample of 65 infected patients and 37 samples of control patients were evaluated for organ dysfunction according to sequential organ failure score criteria.The samples were then grouped and statistically analyzed.Finally,33 patients in the sepsis group were obtained.There were 27 mild infection groups and 30 control groups.6、Plasma was centrifuged and frozen in a-70℃ freezer.7、Thaw those samples stored in-70℃,and test heparin binding protein,procalcitonin and C-reaction protein.8、Caculating related parameters by 21.0SPSS and curve ROC,95% are confidence interval.Results are displayed by average,standard deviation and change range.We adopt one-way analysis of variance and χ2 test.P<0.05 means significant,P>0.05 means insignificant.9、Compare the sensitivity,specificity,positive predictive values,negative predictive values and ROC curve of heparin binding protein,procalcitonin and C-reaction protein.10、Combing heparin binding protein and procalcitonin and C-reaction protein and white blood cell counting,we compare the sensitivity,specificity,positive predictive values,negative predictive values and ROC curve of them.Results1、In the sepsis group,the mean HBP was 117.29±22.81 ng/ml,in the non-sepsis group the mean HBP was 64.79±17.55 ng/ml,and in the control group the mean HPB was 15.70±4.13 ng/ml.Only 12 of 33 patients with sepsis had positive blood culture results.Their average HBP was 128.38±54.01 g/ml,and the average blood HBP of 21 patients with blood culture negative was 97.40±24.85 ng/ml..The sepsis group had higher HBP,PCT,CRP,and WBC than the non-sepsis group and the control group.2、The area under the HBP curve was 0.815,the area under the curve of the PCT was 0.720,the area under the curve of the CRP was 0.323,and the area under the curve of the WBC was 0.579.3.The area under the curve of HBP combined with PCT was 0.992,the area under the curve of HBP combined with CRP was 0.556,and the area under the curve of HBP combined with WBC was 0.868.Conclusion1、The concentrations of heparin-binding protein,procalcitonin,C-reactive protein and leukocyte counts in the sepsis group were higher than those in the non-sepsis group and the control group.2、The sensitivity of heparin-binding protein in the diagnosis of sepsis is the highest,and the specificity of calcitonin is the best.3 、 The area under the curve of heparin-binding protein combined with procalcitonin is the largest. |
