Objective To design and fabricate suction drainage devices and to evaluate its application in temporary abdominal closure. Methods Design and manufacture of different types suction drainage devices.24 homogeneous rats were treated by ligation of the terminal ileum, with low intestinal obstruction induced by abdominal pressure(IAP?12mm Hg).The rats were randomly divided into 2 groups. That is,the forced closure of abdominal group(Group A)and the temporary closure of abdominal cavity group by suction drainage device(Group B).Heart rate(HR),intra-abdominal pressure(IAP),mean arterial pressure(MAP),abdominal perfusion pressure(APP)and other data were measured in the 1 day before operation,1st day after operation,2nd day,3rd day,4th day,5th day,6th day,7th day,for statistical analysis.Pathological examination of abdominal viscera(heart, lung, liver, kidney and intestine)were carried out at the 7th day.At the same time, the clinical data of 6 patients with temporary closure of abdominal cavity by suction drainage devices from August 31,2014 to August 2016 in Chenzhou First People's Hospital were retrospectively analyzed, and the therapeutic effect was evaluated. Results 1.The basic data were not statistically significant(p>0.05);In the forced closure of abdominal group(group A),the heart rate increased significantly at 1 day after the removal of the obstruction of the abdominal cavity(p<0.05);Mean arterial pressure(MAP)decreased significantly from 1 day after surgery(p<0.05);The intra-abdominal pressure(IAP)continued to increase with time, and the abdominal perfusion pressure(APP)decreased. In the use of a suction drainage device for temporary closure of the abdominal group(group B),heart rate unchanged after surgery,intra-abdominal pressure(IAP)decreased significantly on day 1 and decreased gradually with time.At the same time, abdominal perfusion pressure(APP)showed a progressive increase, there was no significant change in mean arterial pressure(MAP).From the 1 day after operation, group B was significantly better than group A,the difference between the data of the two groups was statistically significant(p<0.05).2.Pathological examination: In the forced closure of abdominal group(group A),the rats got severe kidney injury, severe abdominal injury, severe intestinal injury and function of serious in sufficiency. In the use of a suction drainage device for temporary closure of the abdominal group(group B),the rats got slight injury.3.Case analysis: The suction drainage device can be fixed firmly, drainage unobstructed in 6 patients.Removed the suction drainage device after 2 weeks, when the resumption of vital signs smooth operation and II suture incision closed abdominal incision healing.5 cases of surgical incision healing,In 2 cases, the stoma was returned to the abdominal cavity for 6 months.1 case died of multiple organ failure due to posterior wall ulcer perforation. Conclusion Designed and fabricated Temporary drainage device for abdom inal closure. In the surgical treatment of IAH,the Temporary drainag e device for abdominal closure is easy to operate and suitable for use in first aid and reduce the incidence of abdominal compartment syndrome and mortality. So it is valuable in basic hospital and clin ical application.
