Objective By using color doppler ultrasound to detect and locate the position and number of the perforator vessels in perforator flap with descending branch of the lateral circumflex femoral arter,and to verify in intraoperative,to provide the basis for the design of small and medium-sized flap and polyfoliate perforator flap with descending branch of the lateral circumflex femoral arter.Methods Twenty patients were treated with surgery of the perforator flap with descending branch of the lateral circumflex femoral arter in our hospital from December 2015 to December 2016.Those cases were used the color doppler ultrasound(SIEMENS ACUSON Antares)located and marked on the body surface,measuring the diameter of each perforator vessels and recording the number of perforations before surgery.We had confirmed the location of the fascia and record the number of perforator vessels within the range of the flap intraoperative.Besides we made a comparation of the preoperative and the actualsituation during surgery about the lesion.Results(1)There were 45 perforator vessels in the range of flap of the20 patients preoperative,using the color doppler ultrasound.And there were 48 perforator vessels were found within the flap during the surgery,for 40 perforator vessels were accurate positied in the preoperative,and 8were undiagnosed in the preoperative,and 2 were not find in the surgery.The actual distance difference of the perforator vessels more than 5 mm were 3,and the distance differece of 6-20 mm were 2,and 1 had the distance difference above 20 mm compared with the actual position in the surgery.The positive predictive value is 88.89%,and the sensitivity is83.33% of the color doppler ultrasound positioning perforator vessels.(2)The color doppler ultrasound was used to measure the diameter of each perforator vessel preoprative.The flap was designed with the thickest perforator vessel or the thicker perforator vessels as the center,and the flap were survived postoperative.Conclution Color doppler ultrasound can accurately locate the perforator vessels of perforator flap with descending branch of the lateral circumflex femoral arter.It can provide a reference for the design of the small and medium-sized flap and polyfoliate perforator flap with descending branch of the lateral circumflex femoral arter.
