Objective: To explore the relationship between TCD cerebric hemodynamic parameters and lengthening or tortuous basilar artery diagnosed by CTA and to provide the hemodynamic information for early prevention and treatment of posterior circulation ischemia.Methods: This study included 153 cases of CTA and TCD which were checked simultaneously between January 2016 to January 2017 for evaluating the basilar artery at Suzhou municipal hospital.The lengthening and tortuosity of basilar artery were graded according to CTA.There were 144 cases with lengthening or tortuous basilar artery and 9 cases without lengthening or tortuous basilar artery.The TCD cerebric hemodynamic parameters included peak systolic velocity(PSV),mean blood flow velocity(MFV),end diastolic velocity(EDV),pulsatility index(PI),resistant index(RI).The age,hypertension,smoking,total cholesterol,triglyceride,low-density lipoprotein,glucose,homocysteine were compared between basilar artery with lengthening and tortuosity and without lengthening and tortuosity.The relationship between the grading of lengthening and tortuosity of basilar artery and TCD paremeters were analysed.Results:(1)There were 105 males and 39 females in 144 cases with lengthening or tortuous basilar artery,and the male to female ratio was about 2.7 to 1.(2)The age between the subjects without lengthening or tortuous basilar artery and with lengthening or tortuous basilar artery showed no statistically significant difference(P=0.248).The age among the grades of lengthening basilar artery showed no significant statistical difference(P=0.292),the age among the grades of tortuous basilar artery showed no significant statistical difference(P=0.511).(3)The hypertension,smoking,total cholesterol,triglyceride,low-density lipoprotein,glucose,homocysteine showed no significant statistical differences between the subjects with and without lengthening or tortuous basilar artery(P=0.327,P=0.442,P=0.134,P=0.534,P=0.257,P=0.267,P=0.721).(4)The TCD parameters(PSV,MFV,EDV,PI,RI)showed no significant statistical differences among the grades of lengthening basilar artery(P=0.523,P=0.354,P=0.328,P=0.375,P=0.462).The correlation between the TCD paremeters(PSV,MFV,EDV,PI,RI)and the grades of lengthening basilar artery showed no significant statistical differences(P=0.216,P=0.166,P=0.158,P=0.342,P=0.344).(5)The TCD parameters(PSV,MFV,PI,RI)showed significant statistical differences among the grades of tortuous basilar artery(P=0.022,P=0.036,P=0.030,P=0.009).The TCD parameters(PSV,MFV,PI,RI)all showed significant statistical differences between grade 0 and grade 3 of tortuous basilar artery(P=0.018,P=0.007,P=0.006,P=0.003).The correlation between the TCD paremeters(PSV,MFV,EDV,PI,RI)and the grades of tortuous basilar artery showed significant statistical differences(P=0.000,P=0.000,P=0.012,P=0.007,P=0.003).Conclusions:(1)The incidence of lengthening and tortuosity of basilar artery in male were higher than female.(2)Basilar artery with lengthening and tortuosity was not correlative with age,hypertesion,smoking,total cholesterol,triglyceride,low-density lipoprotein,glucose and homocysteine.(3)The hemodynamic of basilar artery changed when it’s extent of tortuosity were obvious,the influence on hemodynamics is not obvious for the lengthening of basilar artery.
