| Objective:To explore the effect of the carotid artery stenosis on the prognosis of patients with ischemic stroke and the predictive value for it.Methods:750 patients hospitalized within 24 hours of onset ischemic stroke were included in the study.Using the method of carotid artery ultrasound to detect the degree of carotid artery stenosis and record the Total Stenosis Score(TSS),and the prognosis data of1 month,3 months and 6 months were summarized. The TSS was divided into five groups(Q1-Q5),and the COX regression model was used to analyze the relationship between TSS and the prognosis of stroke patients, the COX accumulation survival curves of TSS and acute ischemic stroke patients were also drawn at the same time, and the predictive value of TSS for clinical prognosis was evaluated by ROC curve analysis. Results : Univariate survival analysis revealed that Antihypertension 、 TSS were related factors affecting the clinical prognosis of patients with ischemic stroke(P <0.05).This factors were included in the stepwise regression multiariable COX survival analysis. Finally,TSS was the independent factor for the clinical prognosis of patients with ischemic stroke(HR=1.429,95%CI:1.230-1.659,respectively);the ROC curve analysis showed that the area under the TSS curve was statistically significant(AUC=0.753,P<0.05).Conclusion:TSS was the independent risk factor, and the higher the TSS level, the poorer of clinical prognosis of the patient; The method of TSS testing with carotid ultrasound had a better predictive value for clinical prognosis of patients with ischemic stroke. |
